2373145 Primary Facility 2015-05-11 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected The Department received a report of a release from Cabot Oil & Gas at the Butler Well Site. It was reported that the release occurred on Friday May 1st at 1300 hours. A reservoir on a Hayduk Auguring mounted drill rig released 21 gallons. The rig was on site to drill holes for the stations for the gas lines. Two lines on the rig broke in two different spots. When they fixed the lines, the rig took 21 gallons of fluid. The fluid was released to the well pad. There was one spot near the wellheads near the first break and then it moved to the cut slope on the north side of the well pad and started drilling. This is where the second hose broke and this misted the fluid about 30’ to the south of the GPU.
2585443 Primary Facility 2017-04-21 Routine/Complete Inspection No Violations Noted Inspection conducted on 4/17/17. Inspection conducted after concerns expressed by nearby landowner that their water supply could become impacted by nearby natural gas well development activities. Overall site stability looked good. High percentage of silt sock has been removed. All three wells appear to be in production. No violations noted.
2665600 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted Arrived on location at: 09:00. Weather: Clear, cold ~ 30 degs. (3) wells have been drilled on this pad, the Butler, L. 2H, 4H and 6H. All wells were in production at time of inspection. All wells (13x9) and (9x5) annuli were open and venting, plumbed to shared poly vent tank. All wells (5/P) annular vents were shut in to a pressure monitor wireless sending with LED readout. See individual well insp reports for 5 1/2" csg pressures. A Gas check performed at the shared poly annular vent tank showed 75% LEL, 2% CH4.

- See attached pictures for pad entry sign corrections needed to inaccurate well numbers listed. - Overall, wells and pad were found in good condition. No further obvious concerns were noted at time of inspection. No violations noted. - Left location at 10:00.
