It was reported that the release occurred on Friday May 1st at 1300 hours. A reservoir on a Hayduk Auguring mounted drill rig released 21 gallons. The rig was on site to drill holes for the stations for the gas lines. Two lines on the rig broke in two different spots. When they fixed the lines, the rig took 21 gallons of fluid. The fluid was released to the well pad. There was one spot near the wellheads near the first break and then it moved to the cut slope on the north side of the well pad and started drilling. This is where the second hose broke and this misted the fluid about 30’ to the south of the GPU. Violations attached to 115-21665 |

All wells (13x9) and (9x5) annuli were open and venting, plumbed to shared poly vent tank. All wells (5/P) annular vents were shut in to a pressure monitor wireless sending with LED readout. See individual well insp reports for 5 1/2" csg pressures. A Gas check performed at the shared poly annular vent tank showed 75% LEL, 2% CH4.
