2325869 Primary Facility 2014-11-20 Routine/Complete Inspection No Violations Noted Hydraulic fracturing operations were in progress at the location. An employee of Rain for Rent stated to me that casing perforation operations were in progress. I recorded (8.4) millisiemens/cm conductivity in fluid pooled in the recycled water tank battery secondary containment system. The frac tanks in this tank battery which is set up next to the AST, are enclosed with plastic sheeting in an attempt to prevent the fluid in the tanks from freezing. The recycled water is a mixture of mainly flowback and production fluid treated at Cabot’s A&M Hibbard Treatment Facility. I encountered Larry Fulmer, frac superintendent and consultant of Cabot Oil and Gas on the lease while departing from the site. Mr. Fulmer informed me that well stimulation procedures were in progress for the J Hawk 2H well and the fluid on containment exhibiting the elevated conductivity levels mentioned above, will be removed from the containment.
2725148 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted I performed a Routine Complete (RTNC) inspection of the Hawk well site on May 1, 2018. The wells associated with this site are as follows: 115-21650, 115-21651, 115-21652, 115-21653, 115-21654, 115-21655.

The wells were properly labeled.

The cut slope had prior repairs performed, consisting of riprap places in areas of instability of the cut slope. There is now an area of instability above a previous repair wherein a vegetated section is currently slumping into the riprap stabilized area. (See photo) This could potentially be a violation of 102.22(a). It is suggested the issue be repaired as soon as possible to prevent accelerated erosion. Please inform the department within 7 days or upon completion of repairs.

The fill slope is stable and well vegetated. There were no other areas of instability noted.

There are no violations noted.

2787218 Primary Facility 2018-10-09 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Hawk well site.

The day was overcast but dry and in the upper 60s.

The site footprint was approximately 107627 sq. ft.

The wells were properly labeled.

The well pad access road and pad surface was solid and stable.

The fill slope was lush and heavily vegetated with no noticeable stability problems.

The cut slope was vegetated but had rip rap stabilization along much of its length. It was stable however and where there was grass, it was lush and healthy.

The tree plantings at the toe of the fill slope were healthy and viable.

There were no erosional issues noted.

There were no violations noted.
