2321612 Primary Facility 2014-11-12 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 10:15 in response to a reported spill of (5) barrels, (210) gallons of flowback fluid to the ground surface of the tank pad. Brody Webster, Steve Watson and Bob Raup, Environmental Health and Safety Specialists of Cabot Oil and Gas were at the site during the inspection. There was no activity on the tank pad. Flowback procedures for all the wells at the site have recently concluded. Erick Flowback Services was subcontracted by Cabot to conduct flowback procedures. Mr. Webster reported the spill to the Department at 12:18 on 11/10/14. He also reported an additional flowback fluid spill of less than (1) barrel, (42) gallons in volume discovered on the tank pad on 11/10/14. He explained that the flowback fluid spill where (5) barrels impacted the tank pad surface, occurred due to a butterfly valve left open on a frac tank during the transfer of flowback fluid from the well pad to the tank pad.

2677520 Primary Facility 2017-12-15 Routine/Complete Inspection No Violations Noted Arrived on location at: 13:30. Weather: Clear, Cold, ~ 20 degs. A total of (10) wells were present on this pad, the Anderson, R. 1H, 2H, 3H, 4H, 5H, 6H, 7H, 8H, 9H and 10H. All (10) wells were in production at time of inspection. -All wells Prod. Line, Prod. casing, Static and Differential pressures were being monitored by wireless press. monitoring units sending pressures to an onsite Total Flow Control Unit "TFCU". The TFCU Control Panel was password protected, no pressures were accessible at time of inspection. All wells (13x9) and (9x5) annuli were open and venting, plumbed to (2) shared, common vent tanks. These annuli were also equipped with wireless press. monitoring units. A Gas check performed at both shared, common vent tanks showed 100% LEL, 100% CH4. -*Due to the commingling of the wells (13x9) and (9x5) annular vents to these vent tanks, a follow-up inspection will need to be scheduled with Operator to check the (13x9) cemented annuli for any sustained presence of gas. A total of (10) wells are present on this pad, the Anderson, R. 1H, 2H, 3H, 4H, 5H, 6H, 7H, 8H, 9H and 10H. All (10) wells were in production at time of inspection. -All wells Prod. -Onsite in follow-up to initial inspection performed on 12/15/17 which detected 100% LEL and 100% CH4 at both shared annuli vent tanks. In preparation for this follow-up inspection, Cabot has isolated all (10) wells (13x9) annular vents. These vents were isolated and opened to vent to atmosphere at approx. 10:00 this morning. Upon arrival to site I met with Cabot Rep. Aaron Ungst. Gas checks were performed at the (13x9) block & bleed needle valves using an Altair 5X gas meter calibrated this

2714160 Primary Facility 2018-04-04 Site Restoration No Violations Noted I performed a RTNC inspection of the Anderson well site. The access road and pad surface were stable and solid despite overnight rain. The cut slope was well vegetated, greater than 70% perennial coverage. The fill slope was very well vegetated, exceeding the 70% perennial vegetative standard. The wells were properly labeled. There were no temporary BMPs remaining on site. There were no erosional issues observed.

2724254 Primary Facility 2018-04-27 Routine/Complete Inspection No Violations Noted I inspected the Anderson Well Site on 4/27/2018. The wells associated with this site are as follows: 115-21632, 115-21633, 115-21634, 115-21635, 115-21636, 115-21637, 115-21638, 115-21639, 115-21640, 115-21641. The wells were all properly labeled.

The cut and fill slopes were both well vegetated and stable. The access road and pad surfaces were stable and solid. There were no erosional issues apparent.

There were no violations noted.
