2713342 Primary Facility 2018-03-20 Routine/Complete Inspection No Violations Noted Arrived onsite at 14:30. Weather overcast, ~ 38 degs. (5) wells were present on pad, the Phelps 2H, 1H, 1V, 3H and 4H. The 2H well was shut in and the other (4) wells were producing at time of insp. There was also a US Compressor unit onsite. The 1V, 1H & 2H wells were connected to compressor unit at time of insp. Visual insp. of production tanks showed moderate to heavy corrosion and rust staining at 3 out of 6 prod. tanks. (See pics on page 2). Updating and/or maintenance of prod. tanks is needed.

- Inspection of wells showed all (5) wells had the prod. tree top press. "PTTP", prod. casing press. "PCP" and prod. line press. "PLP" being monitored by wireless data sending units and/or analog gauges. - The 2H, 1H & 1V wells had (1) annular vent present labeled (9 5/8"x 5 1/2") venting to a shared poly vent tank. - The 3H & 4H wells had (2) annular vents present labeled (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") venting to a shared poly vent tank. A gas check performed at this vent tank lid opening showed 100% C

2719133 Primary Facility 2018-04-17 Routine/Complete Inspection No Violations Noted Arrived onsite at 11:45. Weather: Raining, ~ 40 degs. (5) wells were present on pad, the Phelps 2H, 1H, 1V, 3H and 4H. The 2H well was shut in and the other (4) wells were producing at time of insp. Onsite in follow-up to perform gas checks at the 3H & 4H wells Surface x Intermediate "(13 x 9)" casing annuli as requested in previous inspection performed on 3/20/18. Upon arrival I met with Chief O&G Rep., Jon Hafer. Jon had isolated and vented these annuli to atmosphere yesterday at ~ 11:00 to vent off any commingled, trapped gas within the annular vent piping. Gas checks performed at these wells (13 x 9) annuli showed: 3H: 0% LEL, 0% CH4; 4H: 80% LEL, 3% CH4.

- The sustained presence of gas detected within the 4H wells cemented Surface x Intermediate annulus is indicative of Defective Cement (78a.86). Since this condition was simultaneously detected by the Dept. and Operator and this well is not part of a active gas migration investigation, no violation will be issued at this time. 2803451 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted This inspection was conducted to confirm Chief's assessment that the project meets the minimum requirements for permit termination. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
