Inspection conducted with Frank Hoehle and Merle Wade, construction inspector. The well permit that has been issued for this pad is the 1H (115-21609). All perimeter BMP’s in place to include red lined additions. Entrance has not been black topped yet. P7 cross culvert has not been installed. Pipe is on-site and will be installed shortly. Pad slopes are approximately 10% complete. Pad is 75% complete. No violations noted.

2469591 Primary Facility 2016-03-30 Routine/Complete Inspection No Violations Noted On 3/30/16, the Department conducted an inspection of the Chief Herbert Well Pad. Only one well has been drilled on this pad, the Herbert 1H 9115-21609). A review of the Well Record revealed that the production string has yet to be drilled. The conductor (20”), the surface string (13 3/8”), and the intermediate string (9 5/8”) have been drilled and set. Date well completed was 12/12/2014. The cellar is approximately 1/3 full of water, with no bubbling present. Pad and slopes stable with good vegetation. No violations noted.

2555313 Primary Facility 2017-01-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:10p.m., the cellar was about a quarter full of water and ice with no bubbling noticed. There were no lines going to or from the well and there was a night cap on the well, all the valves on the well were closed. There were no tanks and production units on site. The well was granted inactive status on 6/24/2016, only the vertical section of the well has been drilled to a depth of 6448'. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:30p.m.

The well was granted inactive status on 6/24/16. The top-hole section of the well was drilled to a depth of 6448'. There were no lines going to or from the well. All the valves were closed and there was a TA cap on top of the well. There were no tanks or production units on the site. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

Drilling has been completed on the 1H and 4H. No stimulation has occurred yet. Overall the site has good stabilization. No E&S issues identified. No violations noted.
