Inspection conducted with Frank Hoehle and Merle Wade, construction inspector. The well permit that has been issued for this pad is the 1H (115-21524). All perimeter BMP’s in place. Discussed with Merle the need to improve the stabilization of the cut slopes at the entrance of the access road. Also discussed re-tracking the top soil pile located below the pad fill slope when conditions permit. Pad is 95% complete. No violations noted.

2339545 Primary Facility 2015-01-27 Routine/Complete Inspection No Violations Noted On 1/27/15, the Department was on site at the Chief Rogers Well Pad to conduct a routine inspection. Well permits that have been issued for this site are the Rogers 1H (115-21524), 2H (115-21525), 3H (115-21526), and the 4H (115-21523). A crew is on the pad actively installing production equipment on the pad. All four wells appear to have been flowed back, and the crews have emplaced the four production units along with a battery of production tanks. Well heads and production equipment is being plumbed together. Production piping is being buried in approximately 4’ deep excavated ditches. This inspection conducted in conjunction with an inspection of the Regency Taylor to Rogers Pipeline construction. Pipeline scheduled to be ready for production sometime mid to late February. Pad is snow covered. No violations noted.
2659985 Primary Facility 2017-11-08 Routine/Complete Inspection No Violations Noted All fill slopes stone stabilized. Today's inspection was conducted as a routine inspection. Site is stable. Photo 1 shows the well pad layout. Photo 2 shows the southeast corner of the well site. No violations noted.
2803579 Primary Facility 2018-11-13 Routine/Complete Inspection No Violations Noted This inspection was conducted to confirm Chief's assessment that the project meets the minimum requirements for permit termination. The Department’s determination is that the site meets the requirements for permit termination. I recommend the NOT be approved.
