Well permits that have been issued for this pad are the 1H (115-21457), 2H (115-21458), 3H (115-21459) and the 4H (115-21460). All perimeter BMP’s in place and include red lined additions. The pad is approximately 80% complete. The tank pad is approximately 15% complete. No violations noted.

2319762 Primary Facility 2014-11-04 Incident- Response to Accident or Event No Violations Noted Andy Mehalko of Cabot Oil and Gas reported the spill incident to the department on 10/30/2014 at 5PM. Mr. Mehalko reported to the Department that a truck driver reported to Cabot that he forgot to disconnect the hose from a frac tank and the hose broke as he drove off. The driver did not report any loss of fluid from the hose. Resource Environmental of Montrose discovered standing fluid was at the site on 10/30/2014. The standing fluid exhibited elevated conductivity. I observed staining and white crystalline residue in several areas of the pad surface. The majority of staining and crystallization I discovered is indicative of the main pathway vacuum trucks would utilize to transport production fluid off the location. Crystallization on the pad surface was also observed in areas potentially not indicative of the main truck route to transport production fluid off the location. The most prevalent area of staining observed on the pad surface is in the southeast section of the pad; N- 41°47’49”; W-75°38”11”.

2513889 Primary Facility 2016-08-25 Routine/Complete Inspection No Violations Noted On 8/25/16, the Department conducted an inspection of the COG Marcho, W&M Pad 1 located in Gibson Township, Susquehanna County. Four wells associated with this pad have been drilled, stimulated, and appear to be in production. The one well that has not been drilled is the 6H (115-21461). Both the well pad and tank pad are present and appear to be stable. Storm water basins with emergency spill ways appear to be in very good condition. A well tender was on-site during the inspection and was working near the well heads. Site is half stone stabilized and half stabilized with perennial vegetation. No violations noted.

The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 596psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:10p.m.

The well was in production. The cellar had water 5' below grade with no bubbling noticed. All the (13x9) and (9x5) annuli were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2682595 Primary Facility 2018-01-12 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All 4 wells, the 1H, 2H, 3H, and 4H appear to be in production. Water within production tank containment, photo 4, is about 4" deep and had a conductivity reading of 3.4 mS. Standing water outside of and around this containment read between 200 and 400 uS. Deduction is that containment is sound. No spills detected. No violations noted.

The well was in production. There was water 4' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
