This pad shares the Kupscznk A Pad access road. The coordinates for the entrance are 41 41 32 and 75 56 21. A drill rig is on the A Pad. No violations noted.

2268364 Primary Facility 2014-04-15 Incident- Response to Accident or Event No Violations Noted NERO EC&B alternate remedial processs reviewed 12/2/15, case closed. During the evening on April 3, 2014 rig hands were attempting to circulate the oil based drilling mud in the tanks when a hose, which was attached to a pump, blew off and released the material. It was reported that the majority of the released material (volume undetermined)was on containment but a few barrels (estimated as 2 bbl) escaped containment and impacted the well pad surface and also traveled to a drainage ditch. Chief stated that no material left the well site. Chief responded with skid steers and vac trucks immediately. On April 4, 2014 Mr. Balum was present for the excavation within the ditch. Mr. Balum explained that M.R. Dirt conducted the excavation and also will be handling the waste disposal. An approximate 30 cubic yards of material were removed from the excavation.Violations associated with this inspection have been attached to the Kupscznk Unit D 3H, Well Permit# 115-21452.
2811694 Primary Facility 2018-10-29 Routine/Complete Inspection No Violations Noted After performing an inspection and administrative review of the site plan, I agree the site conditions match the requirements for the termination of this permit.

While on site, I noticed several tanks in containment being stored. I also noticed a pile of what appeared to be trash left on the pad surface. Operator, please ensure all waste remaining on site has been removed and provide photographic documentation to the Department via email.

On 30OCT2018, I encountered Jon Hafer (Chief OG) while conducting an unrelated inspection. When I inquired about the status and purpose of the tanks being stored on this site, he admitted that he did not know off-hand, but that his best guess was that they were likely associated with Chief's recent pump-down operations on the pad. Mr. Hafer went on to inform me the aforementioned pump-down operations are conducted by Chief to help preserve the integrity of their well casing when hydraulic fracturing is being conducted nearby.
