2717145 Primary Facility 2018-04-11 Routine/Complete Inspection No Violations Noted I inspected the Lathrop Unit well pad on 4/11/2018. I performed a RTNC inspection. The wells associated with this site are as follows: 115-21426, 115-21427, 115-21813. The cut and fill slopes were well vegetated and the access road and pad surface was stable and solid. The wells were properly labeled. There were no erosional issues noted.

There were no violations observed.

2827555 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted I arrived on site at approximately 1422 hours to conduct a site restoration inspection.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, many areas of the site contain exposed erodible material and do not meet the 70 percent permanent perennial vegetative cover requirements for site restoration. Operator, please consult your plan and determine whether these areas should be reseeded/mulched before resubmitting the NOT.
