I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:10p.m.

2554916 Primary Facility 2017-01-20 Incident- Response to Accident or Event No Violations Noted The Department was on site to conduct an investigation into a complaint by the Susquehanna County Conservation District of mud on route 167. (Complaint number 324608) This was in direct traffic with the B. Mead well site, owed by Cabot Oil and Gas (ESX11-115-0138). This site has the following wells associated with it.

The Department arrived on scene at approximately 4pm, after making a drive down the road directly abutting the B. Mead well pad. There was indeed debris out on the road from the pad site due to heavy truck traffic combined with recent heavy rain and frost melt. Frank Hoehle of the Department spoke with Mr. Harold Wells of SGFS on site. Mr. Wells said the road had been swept, and was in the process of being swept again. There was a container on site where the sweeper was emptying into, and the sweeper was on its way back up the road to make another pass as we were speaking. The Department was able to speak to the equipment operator on site, who was spreadin

2556039 Primary Facility 2017-01-25 Follow-up Inspection No Violations Noted Arrived at the site to follow up on the Department’s 1/20/17 Incident Inspection conducted by Frank Hoehle(DEP-WQS). The Department received a complaint (ID# 324608) concerning sediment leaving the well site and being deposited onto Route 167.

I drove along Route 167 on 1/25/2017 and noted that sediment was being deposited along SR 167 heading from the B Mead well site and being tracked onto the roadway heading south. I pulled onto the access road and noted that several trucks were parked on the tank pad. The trucks appeared to be staged for activity elsewhere. It appeared that all trucks leaving the site were heading south.

I contacted Michael Murnin, Steve Watson, and Andy Mehalko all of Cabot Oil & Gas upon returning to the office to report the Department’s findings. I spoke with both Mr. Murnin and Mr. Watson. Mr. Watson explained that they would take care of the sediment leaving the well site. The Department requested that they follow up the phone call with an email detailing their plan to ad

2556508 Primary Facility 2017-01-27 Follow-up Inspection No Violations Noted The Department was on site to conduct a Follow-Up Inspection to the Departments 1/25/17 inspection. At the time of the inspection, I drove along State Route 167 in the vicinity of the B Mead Well Site. I noted that I passed by the GDS street sweeper while inspecting the area. SR 167 was found to be covered in sediment laden water. The mud appeared to have been removed by the street sweeper leaving the sediment laden water. Also, the shoulder of the road was found to have residual sediment that appeared to be frozen in areas or slushy in consistency.
I then inspected the rock construction entrance and noted that the stone did not appear to be the correct size.  Operator, please be advised that the Standard Construction Detail #3-1 Rock Construction Entrance, found in the DEP Erosion and Sediment Pollution Control Program Manual (ESPCGM) states that the rock should be an AASHTO #1.  Operator, please review your road and ensure that it meets the qualifications for a Rock Construction Entrance and make an
2625628 Primary Facility 2017-08-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:15p.m.

The cellar was about three quarters full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 473psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:55p.m.

2684581 Primary Facility 2018-01-19 Routine/Complete Inspection No Violations Noted I inspected the Mead pad on January 19, 2018 for a routine complete inspection. The cut slope and pad surface was solid and stable, and the wells were all properly labeled. The site was snow covered and the area around the wells was recently plowed. There were no erosional issues noted. There were no violations observed
2734692 Primary Facility 2018-05-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water in the cellar .5' below grade and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2790589 Primary Facility 2018-10-17 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the B. Mead well pad. The day was overcast and in the upper 40s. I was originally on site following a complaint regarding dust and mud being dragged onto SR 167. That is detailed in inspection 2790535. This inspection is a routine E&S inspection for the rest of the well site.
    The entrance to the access road was being swept upon my entry to the site and there was a rip rap section of access road prior to the paved apron.  The rip rap was fairly recently applied and not full of mud and debris.  
    The tank pad was muddy but was being adequately contained.  It was recommended to stone the surface to alleviate  any possible future mud issues.
    The well pad was currently under construction to enlarge the pad in anticipation of a near future drilling event.  Areas not being actively worked were covered with mulch and temporary BMPs were in place.  
    The wells already on site were properly labeled.

There were no violations.

2835608 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted I previously performed an inspection (inspection number 2790535) in response to a complaint (number 337613) called in, consisting of mud being pulled onto the road from the B. Mead pad. The weather at the time of inspection was in the upper 30s and the skies were overcast with breaks of sun.

The pad was undergoing a drilling operation at the time of inspection, so truck traffic was constantly heavy. There was ongoing sweeping occurring during the inspection, although the roadside was lightly muddy, not just from truck traffic but also from snowmelt. There was a coating of gravel on the access road. It appeared the operator was addressing the issue of debris carryout at the time of inspection.

There were no violations.
