2309660 Primary Facility 2014-09-02 Routine/Complete Inspection No Violations Noted Nabors Drilling Rig # M 61 is at the active drilling rig on the location. Larry Henry, consultant of Chief Oil and Gas is the company man on the location. The current drilling depth is (6,430.36) feet. The vertical section of the 4H well has just been completely drilled. According to Mr. Henry, drilling of the horizontal section of the well will commence sometime this evening, 9/2/14. Eric McClure, Environmental Specialist of Chief Oil and Gas arrived at the site close to the end of the inspection. I discussed the elevated conductivity readings in standing water on the pad surface and freeboard level in the tank as aforementioned in the inspection with Mr. McClure and Mr. Henry. Mr. McClure and Mr. Henry stated that they would assess the situation immediately upon my departure from the site. They will attempt to remove any standing water at the site and draw down the freeboard level in the tank as soon as possible.
2315238 Primary Facility 2014-10-02 Follow-up Inspection No Violations Noted Arrived at the site at 11:30. Eric McClure, Environmental Specialist of Chief Oil and Gas, who was at the location during the inspection on 10/2/14, contacted me at 8:30 AM on 10/2/14 to report that there was no major evidence of staining or potential contamination of the pad surface discovered upon removing the drilling rig secondary containment liner. During an inspection at the site on 9/2/14, I recorded elevated conductivity readings in standing water on the pad surface. Please see the 9/2/14 inspection report for details. . I did not observe any evidence of contamination of the pad surface during the inspection. I observed what appeared to be lime dust staining the rocks and causing an increase in turbidity in water in the site perimeter ditch at the edge of the pad surface on the northern side of the pad. Conductivity readings were not of concern in this area. Mr. McClure informed me that Chief Oil and Gas is not aware and nor have they reported any spill incidents to the Department during drilling operations at this location.

Namely, the southern portion of the pad that was restored and scarified/seeded has not yet attained adequate vegetative cover and appears to have been recently disturbed. Operator, please ensure the entire site has attained adequate vegetative stabilization before resubmitting this NOT.
