2473428 Primary Facility 2016-04-11 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:15 a.m., the cellar was about three quarters full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 786psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 11:20 a.m.

2612705 Primary Facility 2017-07-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:45a.m.

There was old containment on the ground around the cellar. The cellar was about half full of water and what appeared to be oil on top of the water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 611psi on the production casing. I spoke with Mitch Blascak (well tender for Cabot), he said he would have the cellar cleaned out and the containment picked up. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:40a.m.
