The temporary Sediment Trap #1 has been installed to include the 11’ spill way. Pad construction has not been started yet. No violations noted.

2455481 Primary Facility 2016-03-03 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:50 a.m., the cellar was full of water and ice with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 634psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 12:20 p.m.

2545884 Primary Facility 2016-12-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:14a.m., the cellar was about a quarter full of ice and snow. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 688psi on the production casing and 560psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 9:40a.m.

2632271 Primary Facility 2017-09-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I took a walk around the location and there were no obvious concerns noticed.

2729175 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection on the Reynolds R pad. The wells associated with this site are as follows: 115-21348, 115-21349, 115-21350. The wells were properly labeled.

The pad surface and access road were solid and stable.

The cut slope showed evidence of previous repairs but was otherwise well vegetated. There did not appear to be any areas of instability.

The fill slope had a rill forming midway across the face. (See photo) There was apparently a drain from the pad surface daylighted near the South side of the pad. Downhill from this drain was a somewhat swampy area that appeared to be eroding. At this point the erosion from the small rill and pad drain is minimal but it is recommended this should be addressed promptly. (See photos)

There was an infiltration basin that had an overflow rock apron over plastic. The rock has been pushed off the plastic by the force of runoff. (See photo) It is recommended this be repaired promptly.

There were BMPs remaining at the toe of the fill slope which

The well was in production. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2836614 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of water and ice, there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
