Well site stability is good. E & S inspections up-to-date. A crew is on-site conducting flow back operations of the 4H, the only well drilled on this site. The purpose of the flow back operation is to conduct production logging. There are no plans to place this well into production in the near future. A tank farm and flare stack are in place and are being utilized as part of this operation. Containment looks good. No violations noted.
2337705 Primary Facility 2015-01-16 Follow-up Inspection No Violations Noted An Integrated Production Services coil tubing unit is on the location. Flowback procedures are still in progress for the Spencer Unit 4H well. While in the process of surveying secondary containment, the equipment and frac tanks associated with the flowback procedures and the ground surface of the Pad, the Department observed (2) light towers and a heating unit staged on containment systems which are not completely secure. Frac Sand was also observed on the pad surface near the flare stack and on the fill slope of the pad. We inquired with the company man the reason as to why the frac sand was on the pad surface and fill slope. He stated that the sand was used during hydraulic fracturing operations for the Spencer Unit 4H well and remnants of the sand were left at the site. The Department had suspected the possibility a slug of fluid mixed with the sand may have escaped out of the top of the flare stack during natural gas flaring of the well during flowback operations. The company man informed us that this did not occur.

2542836 Primary Facility 2016-11-18 Routine/Complete Inspection No Violations Noted 11/18/16, the Department conducted an inspection of the Chief Spencer Well Pad located in Lenox Township, Susquehanna County. The inspection was in response to a complaint that gas well venting was occurring on this pad. The Department arrived on site and found no active activity occurring on this pad beyond steady state operations. Of the seven wells that have been permitted for this site, only 2 wells have been drilled on this pad, the 3H (115-21338) and the 4H (115-21339). The 3H has only had the Surface and Intermediate casings installed, with inactive status granted on 5/9/16. The 4H has been drilled, stimulated, flowed back, and appears ready to be placed into production, with all associated production equipment installed on-site and tied in. The 4H appeared to be shut in during this inspection. Both wells have their annuluses vented to a single poly tank, with no obvious venting occurring on the pad.
