2322226 Primary Facility 2014-11-13 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 13:00 in response to an estimated (15-20) gallon spill of production flowback fluid. Brody Webster, Environmental Health and Safety Specialist of Cabot Oil and Gas reported the spill incident to the Department on 11/12/14 at 19:37. The report states that the spill occurred while a truck was leaving the site and is already cleaned up. Bob Raup, EH&S Specialist of Cabot was at the site during the inspection. Mr. Raup stated that the spill occurred around 7PM on 11/12/14 due to a mechanical malfunction in a vacuum truck. A valve may have popped off on the truck, releasing the fluid to the surface of the access road. The flowback fluid is untreated flowback fluid from the wells at this location. Currently, well flowback procedures are in progress at the site.
2325719 Primary Facility 2014-11-20 Follow-up Inspection No Violations Noted Flowback procedures were in progress at the site. Cabot was also in the process of installing production equipment and in preparation for the production phase of operations. I met with Kristina Zamorski of Arcadis during the inspection. Cabot submitted an alternative waste management practice (OG-71) application to the Department on 11/3/14 for an on-site mobile pretreatment system for hydraulic fracturing flowback fluid. The application was acknowledged by the Department on 11/5/14. The secondary containment on the tank pad appeared to be in good condition.

2397070 Primary Facility 2015-08-07 Administrative/File Review No Violations Noted This administrative inspection has been created to document Cabot’s remediation of a flowback fluid spill of an estimated (20-25) gallons which occurred on the access road of the B Bishop well pad as documented in the B Bishop 11/13/14 and 11/20/14 inspection reports. A review of the remediation summary report prepared by Resource Environmental and submitted by Cabot on 8/4/15 indicates that the remediation was adequate. No further action is required of Cabot at this time.
