Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2198441 Primary Facility 2013-08-26 Routine/Complete Inspection No Violations Noted The Department was on site at the Cabot Payne, D. P1 Well Site to conduct a Preoperational Inspection on 8/26/13. Well permits that have been issued for this pad are the 2H (115-21305), 3H (115-21304), and the 8H (115-21306). All perimeter BMP’s in place. Pad is approximately 98% complete. Site has been constructed as per plan. No violations noted.
2215059 Primary Facility 2013-10-22 Drilling/Alteration Pending 20 set at 113'
2215063 Primary Facility 2013-10-18 Drilling/Alteration Pending Well drilled however noticed 100% LEL in cellar. Shut in well and built up pressure on 13 3/8 X 9/8 up to approx 269psi. Currently venting. Skid rig over to well and ran logs on 5 1/2 string. Noted possible deficiency at 2250'. Plan to run logs on 9 5/8 to determine if cement is deficient.

2407903 Primary Facility 2015-09-16 Routine/Complete Inspection No Violations Noted The Department was on site to conduct an E&S inspection of the Payne Well Pad in Harford Twp. The pad was in good shape, no erosional issues were apparent and the site was stable.
2472568 Primary Facility 2016-04-06 Routine/Complete Inspection No Violations Noted
2483838 Primary Facility 2016-05-13 Routine/Complete Inspection No Violations Noted
2497215 Primary Facility 2016-06-29 Routine/Complete Inspection No Violations Noted On 6/29/16, the Department was on site at the COG Payne D. Pad 1 to conduct a routine inspection. The pad has good permanent stabilization, with approximately 70% vegetative stabilization and 30% rock stabilization. Filter sock at the site could probably be removed at this point. All 3 wells on the pad, the 2H, 3H, and 8H appear to be in production. All 3 cellars ½ full of water with no bubbling observed. E & S inspections up-to-date. The Department highly recommends that the access road and associated ditches/culverts be cleaned and re-sculpted to maintain efficiency which is critical since the access road has a rather steep grade and sharp turn. Overall, the site looks good. No violations noted.
2554811 Primary Facility 2017-01-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:20p.m., the cellar was about a quarter full of water and ice with no bubbling noticed. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 596psi on the production casing and 570psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 12:55p.m.

2643084 Primary Facility 2017-10-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annulus vent pipes were connected together and going to the same tank. There was containment near the 2H well and it appears tubing was recently put in the 2H. The containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2748228 Primary Facility 2018-07-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
