2497216 Primary Facility 2016-06-29 Routine/Complete Inspection No Violations Noted On 6/29/16, the Department was on site at the COG Payne D. Pad 1 to conduct a routine inspection. The pad has good permanent stabilization, with approximately 70% vegetative stabilization and 30% rock stabilization. Filter sock at the site could probably be removed at this point. All 3 wells on the pad, the 2H, 3H, and 8H appear to be in production. All 3 cellars ½ full of water with no bubbling observed. E & S inspections up-to-date. The Department highly recommends that the access road and associated ditches/culverts be cleaned and re-sculpted to maintain efficiency which is critical since the access road has a rather steep grade and sharp turn. Overall, the site looks good. No violations noted.
2554816 Primary Facility 2017-01-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:20p.m., the cellar had water and ice at the bottom with no bubbling noticed. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 575psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 12:55p.m.

2643088 Primary Facility 2017-10-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annulus vent pipes were connected together and going to the same tank. There was containment near the 2H well and it appears tubing was recently put in the 2H. The containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2748226 Primary Facility 2018-07-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 2' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
