Still in the process of clearing and grubbing the well pad site. E&S BMP’s are being installed as work progresses. Current stabilization is adequate. This inspection does not qualify as a preoperational inspection.

Company decided because of valve position as primary barrier to call in wild well control company to assess and address situation. Incident was the result of a failure of the gate valve on the Seaboard well head as a result of a small crack or hole of approximately one half inch in length. Department staff documented combustible gas concentration near the well head was 100% of the Lower Explosive Limit (LEL).

2468286 Primary Facility 2016-03-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40 p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 542psi on the production casing. I took a walk around the location and noticed some oil residue around the edge of some water holes on the site. I contacted Steve Watson of Cabot Oil & Gas and he said it will be taken care of.

I left the site at 13:20 p.m.

2637475 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted Arrived on location at 09:00. Weather: Overcast, mild. Approx. 64 degs.

A total of (3) wells have been drilled on this pad, the Houston, J 1H, 2H and 8H wells. All wells were in production at time of inspection. All wells (13x9) and (9x5) annuli were open and venting, plumbed to common vent tank. Gas check performed at vent tank showed 6% LEL, 0% CH4. All wells were producing through 5 1/2" prod. casing. Prod. casing pressures were being monitored by wireless sending units to onsite Total Flow Control Unit "TFCU". See individual well pressures and flow rates listed below:

- 1H: SP= 472.6 psi, DP= 66.6 psi, FR= 2746 MCF/day, CP= 470.0 psi. - 2H: SP= 473.1 psi, DP= 102.9 psi, FR= 4560 MCF/day, CP= 445 psi. - 8H: SP= 475.8 psi, DP= 163.2 psi, FR= 2906 MCF/day, CP= 462 psi.

Overall, wells and pad were found in good condition. No obvious concerns noted at time of inspection. No violations noted.

2682191 Primary Facility 2018-01-12 Routine/Complete Inspection No Violations Noted Arrived on location at 08:00. Weather: Overcast, light rain, ~ 60 degs. A total of (3) wells have been drilled on this pad, the Huston, J 1H, 2H and 8H wells. All wells were in production at time of inspection. All wells (13x9) and (9x5) annuli were open and venting, plumbed to a common, shared vent tank. Gas check performed at vent tank showed 0% LEL, 0% CH4. - All wells were producing through 5 1/2" prod. casing. Prod. casing pressures were being monitored by wireless sending units sending data to onsite Total Flow Control Unit "TFCU". See individual well pressures and flow rates listed below: (CP= prod. casing press., SP= static press., DP= differential press.) - 1H: CP= 544.5 psi., SP= 545.1 psi., DP= 45.3psi. - 2H: CP= 514.9 psi., SP= 545.0 psi., DP= 84.6 psi. - 8H: CP= 530.2 psi., SP= 548.4psi., DP= 136.8psi. - Overall, wells and pad were found in good condition. No obvious concerns noted at time of inspection. No violations noted. Left location at 09:30.
