2180200 | Primary Facility | 2013-06-25 | Routine/Complete Inspection | No Violations Noted | The Department was on site at the Houston J Well Site for a routine inspection. Well permits that have been issued for this pad are the 1H (115-21272), 2H (115-21273), 7H (115-21274), and the 8H (115-21275). The access road is 50% complete. Still in the process of clearing and grubbing the well pad site. E&S BMP’s are being installed as work progresses. Current stabilization is adequate. This inspection does not qualify as a preoperational inspection. |
2241778 | Primary Facility | 2014-01-05 | Incident- Response to Accident or Event | Violation(s) Noted | Well control incident for Huston J1- resulted in release of unknown quantity of gas and fluids to well pad and potentially other ground surfaces. |
2244794 | Primary Facility | 2014-01-06 | Drilling/Alteration | Outstanding Violations - No Viols Req'd | Well control incident. Gate vavlve on fract head faulted 1/5/2014. Onsite at 7AM (1/6/14)....waiting on 7:30 safety meeting to discuss incident with WWC. WW plans on nippling up two hydraluic vavles on top of fract head to divert gas so that gate vavle can be repaired. 10:30AM - attaching well head, open up valves. Gas diverted thru replacment stack. 12PM...well under control, stack to be replaced. |
2257196 | Primary Facility | 2014-03-27 | Drilling/Alteration | Violation(s) Noted | Responded to well control incident. Primary gate valve revealed crack or hole in top of seat body. Company decided because of valve position as primary barrier to call in wild well control company to assess and address situation. Incident was the result of a failure of the gate valve on the Seaboard well head as a result of a small crack or hole of approximately one half inch in length. Department staff documented combustible gas concentration near the well head was 100% of the Lower Explosive Limit (LEL). |
2257201 | Primary Facility | 2014-03-25 | Routine/Complete Inspection | No Violations Noted | piping in well |
2258225 | Primary Facility | 2014-03-27 | Routine/Complete Inspection | Outstanding Violations - No Viols Req'd | Well control incident-arrived on site approx 1000hrs. master valve on well has crack ,is closed but leaking small amount of gas. no well shut off below this gate. uncontrolled methane into atmos. no fluid plans formulated to kill well and repair - replace gate valve. |
2259127 | Primary Facility | 2014-03-29 | Drilling/Alteration | No Violations Noted | 3/29/14 insp found adequate corrective steps taken in response to 3/27/14 WCI, no fluids lost, well head secured and repaired/replaced, well placed back into production on 3/29/14, past viols closed. |
2369778 | Primary Facility | 2015-05-14 | Routine/Complete Inspection | No Violations Noted | |
2468286 | Primary Facility | 2016-03-28 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 12:40 p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 542psi on the production casing. I took a walk around the location and noticed some oil residue around the edge of some water holes on the site. I contacted Steve Watson of Cabot Oil & Gas and he said it will be taken care of.
I left the site at 13:20 p.m. |
2637475 | Primary Facility | 2017-09-19 | Routine/Complete Inspection | No Violations Noted | Arrived on location at 09:00. Weather: Overcast, mild. Approx. 64 degs.
A total of (3) wells have been drilled on this pad, the Houston, J 1H, 2H and 8H wells. All wells were in production at time of inspection. All wells (13x9) and (9x5) annuli were open and venting, plumbed to common vent tank. Gas check performed at vent tank showed 6% LEL, 0% CH4. All wells were producing through 5 1/2" prod. casing. Prod. casing pressures were being monitored by wireless sending units to onsite Total Flow Control Unit "TFCU". See individual well pressures and flow rates listed below: - 1H: SP= 472.6 psi, DP= 66.6 psi, FR= 2746 MCF/day, CP= 470.0 psi. - 2H: SP= 473.1 psi, DP= 102.9 psi, FR= 4560 MCF/day, CP= 445 psi. - 8H: SP= 475.8 psi, DP= 163.2 psi, FR= 2906 MCF/day, CP= 462 psi. Overall, wells and pad were found in good condition. No obvious concerns noted at time of inspection. No violations noted. Left location at 11:00. |
2682191 | Primary Facility | 2018-01-12 | Routine/Complete Inspection | No Violations Noted | Arrived on location at 08:00. Weather: Overcast, light rain, ~ 60 degs. A total of (3) wells have been drilled on this pad, the Huston, J 1H, 2H and 8H wells. All wells were in production at time of inspection. All wells (13x9) and (9x5) annuli were open and venting, plumbed to a common, shared vent tank. Gas check performed at vent tank showed 0% LEL, 0% CH4. - All wells were producing through 5 1/2" prod. casing. Prod. casing pressures were being monitored by wireless sending units sending data to onsite Total Flow Control Unit "TFCU". See individual well pressures and flow rates listed below: (CP= prod. casing press., SP= static press., DP= differential press.) - 1H: CP= 544.5 psi., SP= 545.1 psi., DP= 45.3psi. - 2H: CP= 514.9 psi., SP= 545.0 psi., DP= 84.6 psi. - 8H: CP= 530.2 psi., SP= 548.4psi., DP= 136.8psi. - Overall, wells and pad were found in good condition.
