Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2180075 Primary Facility 2013-06-25 Routine/Complete Inspection No Violations Noted The Department was on site at the Adams J Well Site to conduct a Preoperational Inspection. Well permits that have been issued for this pad are the 1H (115-21261), 2H (115-21262), 4H (115-21263), and the 6H (115-21264). Inspection conducted with George Bishop of GDS. All perimeter BMP’s in place. Pipeline is actively being installed across the access road approximately 600’ from the entrance. In process of hydroseeding the pad with approximately 65% complete. Pad is approximately 95% complete.
2211822 Primary Facility 2013-10-09 Incident- Response to Accident or Event No Violations Noted Violations associated with this inspection can be referred to in the J Adams 1 inspection report. Permit number is 115-21261. E-facts identification number is 2211789.
2235833 Primary Facility 2014-01-13 Drilling/Alteration No Violations Noted
2240631 Primary Facility 2014-01-29 Drilling/Alteration No Violations Noted
2245762 Primary Facility 2013-12-16 Incident- Response to Accident or Event Violation(s) Noted An estimated (50) to (75) barrels of recycled flowback fluid flowed out of a man way door on Rain for Rent frac tank number (257755) onto the tank battery secondary containment in the northeast corner of the Pad. A valve was not locked on the tank as it was in the process of being filled. A portion of the fluid escaped secondary containment and impacted the surface of the ground at the site. The impact to the ground surface is apparently due to compromised liner integrity and overflow of the fluid on containment to the ground surface. Compromised liner integrity was observed near the tank battery containment berm. I recorded elevated conductivity readings, over range on my meter on and off containment on the Pad and in an interceptor trench just the below the Pad dug to collect the fluid as a subsurface release is apparent.
2507141 Primary Facility 2016-08-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:05p.m., the permit number on the well was unreadable, the cellar was about half full of water with no bubbling noticed but had debris in it, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 519psi on the production casing. I took a walk around the location and noticed a role back container full of containment that was not on containment and not sealed. I contacted Steve Watson of Cabot Oil and Gas to let him know of my inspection and he told me he would get everything taken care of.

I left the site at 13:45p.m.

2533317 Primary Facility 2016-11-01 Routine/Complete Inspection No Violations Noted On 11/1/16, the Department conducted an inspection of the COG Adams J Well Site located in Harford Township, Susquehanna County in response to a nearby malodor complaint. The Department arrived on-site at approximately 10:00 AM. There are 2 wells drilled and in production on this pad, the 4H and the 6H. The 1H and 2H were proposed but never built. The pad only has the standard equipment associated with steady state production. No other activity is occurring at this pad. I spent approximately 5 minutes at each corner of the well pad and did not detect any malodors. No E&S issues were observed during this inspection. No violations noted.
2605588 Primary Facility 2017-06-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:55p.m.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 494psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:30p.m.

2708489 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was ice 4' below grade in the cellar and there was no gas detected from there to the surface. The (13x9) and (9x5) annuli vent pipes are connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2750981 Primary Facility 2018-07-09 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as part of an investigation into a nearby water supply complaint in which the water well was impacted by a heavy sediment load on 7/3/18. As Photo #2 shows, new wells have recently been spud on this pad. A review of the most recent Department Drilling Inspection, Inspection Record No. 2748434 found that conductor pipe was installed on 7/3/18 for 7 wells, the 1H, 2H, 3H, 5H, 7H, 8H, and the 10H. The conductors were all drilled to a depth of approximately 105'. During the 7/3/18 inspection, the inspector noted that 3 Rigs were being operated, one auger rig and two air rigs. No E&S issues were identified. No violations noted.
2850436 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Present during the inspection was Cabot Site Supervisor Steve Gillman. Stimulation activities have been completed and site is currently being demobilized. Production equipment installation is also occurring. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
