2179924 Primary Facility 2013-06-11 Drilling/Alteration No Violations Noted Spudder rig on location
2188557 Primary Facility 2013-07-26 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Arrived at the site at 13:45 in response to a mineral oil spill. Patterson UTI Drilling Rig #257 is the active drilling rig on location currently driling the 3H well at (371) feet. Rich Adams, Environmental Compliance Manager of Chief Oil and Gas, contacted the Department on 7/26/13 to report a release of an estimated (20) barrels of oil to secondary containment that occurred in the mid-morning of 7-25-13. The Marcellus incident report indicates the spill occurred as a result of a valve left open on a tank. A hole was found in the secondary containment and the plastic pad cover under the containment. An estimated (84) gallons impacted the surface of the ground on the Well Pad and (18) barrels of the mineral were recovered. A vacuum truck was immediately dispatched to the site to gather oil on standing water under the plastic. The excavation commenced down to clean soil on the Well Pad and the spilled material did not leave the Well Pad. A full spill cleanup report is pending from the company. NERO EC&B reviewed adequate ARP 11/1/13.
2188604 Primary Facility 2013-07-22 Routine/Complete Inspection No Violations Noted Violations and specifics associated with this inspection can be referred to in the Oliver 1H inspection report. Permit number is 115-21212.
2188607 Primary Facility 2013-07-23 Follow-up Inspection No Violations Noted Specifics associated with this inspection can referred to in the Oliver 1H inspection report. Permit number is 115-21212.
2190205 Primary Facility 2013-08-07 Incident- Response to Accident or Event Violation(s) Noted Arrived at the site at 10:24 in response to a diesel fuel spill. Patterson UTI Drilling Rig #257 is the active drilling rig on location. The current drilling depth is (9,148) feet. The Oliver 3H well drilling had been completed. Rich Adams, Environmental Compliance Manager of Chief Oil and Gas, contacted the Department on 8/6/13 to report a release of an estimated (300) gallons of diesel fuel. The Marcellus incident report indicates a fuel filter sight glass broke resulting in a spill of approximately (100) gallons of diesel fuel off containment. The diesel fuel overflowed the berm next to the rig mat, flowed onto the surface of the ground of the Well Pad and pooled in a pad side ditch. The diesel fuel was immediately where standing product was observed and absorbents were utilized to contain the material. The amount of the material recovered is unknown. An excavation is scheduled to take place in the afternoon of 8/6/13. A photoionization detector will be used to determine the extent of the excavation. Spill amount was (948) gallons.
