The site is less than 5 acres. The site is stable, vegetation coverage around the pad and access is greater than 70% perennial growth and stable, and no erosion issues were observed at the time of the inspection. The Post Construction Stormwater Management (PCSM) controls appear to be installed according to the 2010 E&S and restoration plans. The pad PCSM controls consist of a drainage channel around the pad that drains on both sides to a sediment basin and appear to be functioning properly. The access road appears to be in good condition and the PCSMs (drainage channel and culverts, and etc) are in place according to the plans referenced above with no apparent problems.

2491264 Primary Facility 2016-06-02 Follow-up Inspection No Violations Noted I conducted an inspection of the site accompanied by Scott Kuchta, DEP O&G WQS. A coiled tubing unit is on the location. Shortly after our arrival at 10:30, wet met with Gilbert Blevins, site company man, consultant of Chief. Mr. Blevins stated the following information. "We just finished drilling out frac plugs on the 3H well. We started to drill out the plugs yesterday. There were (12) frac plugs drilled out. The plugs were drilled out at a rate of (15) barrels an hour, (3) barrels in 3.25 hours. The Rain for Rent frac tanks contain water from the frac plug drill out process and water used during the frac. The frac job was completed last week. All of the wells are currently shut in."

Chief submitted a stimulation notification to the Department on 5-18-16. The notification indicated stimulation of the Stone Unit 3H would commence on 5/24/16 at 6:00 AM. According to our records, there are (8) wells drilled on the pad. The 2H, 1V, 5H and 4H wells were in production. The 6H, 7H and 8H have been
