2119744 Primary Facility 2012-11-26 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Arrived at the site at 10:00 in response to a release of approximately (20) gallons of production fluid that occurred on 11-21-12 at 4:30 AM. Mr. Webster reported the spill to the Department on 11/21/12 at 12:54 PM. According to the incident report, the result of the release was due to a hammer union coming apart on the line to the production tank associated with the E Bunnell 6H Well. Cabot was able to determine the quantity of the release based on the alarm and the amount of fluid that has been sent to the production fluid tank. G.L Parrish, Cabot and Resource Environmental Inc. arrived at 6AM on 11/21/12 to rig down the GPU. GDS began excavation at 2PM on 11/21/12 under the supervision of Resource. A total of (20) cubic yards of impacted material has been removed from the site. The impacted material was placed into (2) roll-off containers, each half full.

2125850 Primary Facility 2012-11-26 Incident- Response to Accident or Event No Violations Noted Cabot consultant noted a stained area around the production tanks. At this time the consultant took soil conductivity readings which were noted to be “suspect.” Production tanks were rigged down and moved. The production tanks are now situated on liner. On 11/23/12 the stained area was excavated and samples were collected. The excavation measured an approximate 34’X25’X3.5”. A total of 30 cubic yards of material were removed. The waste disposal will be handled by Diaz and it will be taken to Tunnel Hill Landfill in Ohio. Cabot employees suspect the issue occurred due to hoses that were rigged on the top of the production tanks to offload the fluid. Possible that the lines were not being cleared sufficiently, and fluid left in the lines had been dripping over time in the area causing the stain.Mr. Webster stated that Cabot Oil & Gas removed the hoses from the top of the tank and will now have the truck drivers attach the hose upon arrival and remove the hose at departure to prevent future occurrences. I took a walk around the location and everything looked OK.

I left the site at 13:30 p.m.

2440731 Primary Facility 2015-09-08 Routine/Complete Inspection No Violations Noted Inspection in conjunction with the notice of termination inspection of the Vandermark Pipeline. No issued observed at the time of the inspection.
2489060 Primary Facility 2016-05-26 Site Restoration No Violations Noted Inspection of the Bunnell E P2 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices are in place according to approved plans and appear to be functioning properly.

However, it appears all temporary erosion and sedimentation BMPs have not been removed from the site - a triple filter sock sediment trap remains on the east side of the pad. There are also areas that are not meeting the permanent stabilization requirements and are a cause for concern regarding erosion:

1. The former access road from the well pad to the restored tank pad has little to no vegetation and is sloped enough to be a concern regarding erosion. 2. The south fill slope is sparsely vegetated in areas and has tracks with no vegetation from some type of heavy equipment running from the well pad surface to the bottom of the slope in several areas. 2541671 Primary Facility 2016-12-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:30p.m., the cellar was about half full of water with no bubbling noticed. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 609psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:05p.m.

2557863 Primary Facility 2017-02-02 Routine/Complete Inspection No Violations Noted The Department was on site in response to the receipt of a Restoration Report. At the time of the inspection, the Department noted that the E&S Inspection Log indicated that Jeff Look (TEC) conducted the most recent inspection on1/4/2016 and that it was a post precipitation inspection.

PCSM BMP's included a vegetated filter strip south of the well site. The site appears to be stable.
