Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2105633 Primary Facility 2012-09-19 Drilling/Alteration No Violations Noted Patterson 263. Running 13 3/8" casing now. Walked site with Co. man. Secondary containment in place and in good condition. Having safety meeting while on site prior to running casing. Haliburton on standby to cement. No violations observed at this time.
2115091 Primary Facility 2012-11-07 Drilling/Alteration No Violations Noted Walked pad pre-frac suggested enhaced containment
2125271 Primary Facility 2012-12-05 Routine/Complete Inspection No Violations Noted Production crews on site. Flow back of wells is complete. Crews on site are breaking down flow back equipment. Secondary containment in place. Well tags are present. Site Id sign correct. No violations observed.
2246179 Primary Facility 2014-02-04 Incident- Response to Accident or Event Pending Arrived at the site at 13:45 in response to a spill of (15) gallons of production fluid. The spill occurred as a result of the dump line freezing.
2271279 Primary Facility 2014-05-16 Administrative/File Review Viol(s) Noted & Immediately Corrected
2691995 Primary Facility 2018-01-29 Routine/Complete Inspection No Violations Noted Arrived on location at 11:30. Accompanied by DEP OGI, Ulyana Trynovich. Weather: Clear, cold, ~ 25 degs. (2) existing wells were present and in production, the Flower, T. 1H and 2H. Both wells (13x9) and (9x5) annuli were open and venting, plumbed to a common, shared vent tank. A Gas check performed at vent tank showed 100% LEL, 20% CH4. *Due to the commingling of the (13x9) and (9x5) annular vents to these vent tanks, a follow-up inspection will need to be scheduled with Operator to check the (13x9) cemented annuli for any sustained presence of gas. Please contact me when these (13x9) vents can be separated or isolated and rechecked. (kenkennedy@pa.gov). Both wells Prod. casing, Static and Differential pressures were being monitored by wireless sending units sending data to onsite Total Flow Control Unit "TFCU". See individual well pressures and flow rates listed below:

(CP= prod. casing press., SP= static press., DP= differential press.) - 1H: CP= 573.2 psi., SP= 557.3 psi., DP= 94.2 psi. - 2H: CP= 581.2

2697689 Primary Facility 2018-02-14 Follow-up Inspection No Violations Noted Arrived on location at 08:15. Weather: Overcast, cold, ~ 32 degs. (2) existing wells were present and in production, the Flower, T. 1H and 2H. Onsite in follow up to previous insp. performed on 1/29/18. Upon arrival I met with Cabot Rep., Aaron Onks. As requested in previous inspection, Mr. Onks had isolated both wells (13x9) and (9x5) annuli and opened vents to atmosphere at ~ 8:00 this morning. Gas checks performed at these annular vents showed:

- Flower, T. 1H: (13x9): Initial check 100% LEL, 2% CH4, diminishing to a Final check of 0% LEL, 0% CH4. (9x5): 0% LEL, 0% CH4. - Flower, T. 2H: (13x9): Initial check 100% LEL, 4% CH4, diminishing to a Final check of 0% LEL, 0% CH4. (9x5): 100% LEL, 100% CH4 sustained.

It appears the initial gas hits detected at both wells (13x9) annuli were from remnant, commingled, gas trapped within common vent lines which was sourced by the Flower, T. 2H wells (9x5) actively venting annulus. The gas venting at this (9x5) annulus is expected due to the production casing ceme
2713984 Primary Facility 2018-03-22 Drilling/Alteration No Violations Noted Matt Shope, DEP O&G WQS and I arrived at the site at 11:23 to conduct an inspection. The weather conditions were partly cloudy skies, quite breezy and cold with temps in the upper 30's. Precision Drilling Rig # 548 is at the site. Mr. Shope and I went inside of the company man trailer in search of the company man shortly after arrival. He was not inside the trailer. According to the white board on the wall in the trailer, Precision is currently drilling the T Flower 008 well at depth of (2,805.20) feet. Shortly after reviewing the board and computer screen containing information relevant to drilling of the well and reviewing drill cuttings waste manifests, Mr. Shope and I proceeded to conduct an inspection of the site. We had observed that a portion of the secondary containment liner was not secured in (3) separate areas of the site. See photos. We eventually met Marcus Cormier, site company man, consultant of Cabot. We had a brief discussion regarding our site inspection near the rig and where additional we
2723274 Primary Facility 2018-04-20 Routine/Complete Inspection No Violations Noted I was accompanied by Ken Kennedy (OGI) and Casey Baldwin (WQS/ET). Precision Drilling Rig 546 is currently operating at the site. Prior to conducting the inspection, we met with Marcus Cormir (Company Man). The 12H well was being drilled at time of the inspection. There were no violations noted. If you have any questions, please contact me.
2756222 Primary Facility 2018-07-11 Routine/Complete Inspection No Violations Noted I conducted the inspection accompanied by Jon Sassi (EPS) and Casey Baldwin (WQS/ET). At the time of the inspection, the 11H Well plugs were being drilled out, a Coil Tubing Unit was on the 6H Well, and another service rig was being prepared for the 3H Well. Additional information related to this inspection may be referenced in e-Facts Inspection ID: 2751430. If you have any questions, please contact me.
2798834 Primary Facility 2018-10-16 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Casey Baldwin, DEP O&GWQS out of the Scranton District Office and I arrived at the site 11:46 in response to a reported spill of an unknown volume and time of a spill of hydraulic fluid to the pad surface. The weather conditions are mostly sunny skies, breezy and brisk, with temperatures in the mid 40's. On 10/16/18 at 8:56, Brody Webster, Environmental Health and Safety Supervisor of Cabot Oil and Gas contacted me to report the spill incident. He stated the following during our phone conversation. "A well tender discovered staining on the pad surface earlier this morning , 10/16/18, as a result of a spill of hydraulic fluid. A coil tube unit recently performed clean out operations on the T Flower 2H well. Hydraulic fluid was released out of the unit and impacted the pad surface. We are not able to calculate the actual volume of the fluid released at this time, but believe it is not more than (5) gallons, and consider this a small spill. GasSearch Drilling Services (GDS) is at the site in the process of
