2125873 Primary Facility 2012-11-26 Incident- Response to Accident or Event Violation(s) Noted This is also a violation of 691.402, 6018.610, and 3218.2(a).he operator was beginning to disassemble Precision Rig #531 and move everything off site. The Genset tank was moved from containment to the southeastern corner of the pad in an area that did not have containment. The Genset tank has an 8,500 capacity and was noted to have 4,000 gallons at the time of the release. The tank was then going to be emptied in preparation of the move. Maximum Fuel Company arrived onsite to remove the fuel from the tank. The driver hooked his equipment up to the top of the tank but forgot to open the vent line. The truck’s hose pressured up and the quick release ball valve blew apart from the manifold releasing 10-15 gallons of diesel fuel to the surface of the pad. Cabot employees determined that the fluid that was released was from what was in the hose at the time of the disconnection. GDS was on site to begin excavation at 3:30PM. The excavation measured 25’X12’X1”. Resource collected samples.
2128892 Primary Facility 2013-01-02 Incident- Response to Accident or Event No Violations Noted The Department was on site at the Rag Apple LLC well site to conduct an Incident Inspection in response to a self-reported release. The Department met with Brody Webster (Environmental, Health & Safety Specialist- Cabot Oil & Gas). Mr. Webster explained that on 12/24/12 a GDS employee was fueling a heater that was situated on containment. The tanks overflowed onto containment then onto the well pad. It was originally estimated that 10 gallons of off road diesel were lost however a new estimate of 25 gallons was given at the time of the inspection.

2465460 Primary Facility 2016-03-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:10 a.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 650psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:50 a.m.

2545904 Primary Facility 2016-12-19 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:10p.m., the cellar was full of ice. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 600psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:45p.m.

2547847 Primary Facility 2016-12-01 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection of the Cabot Rag Apple well site (ESX11-115-0169) The wells associated with this site are as follows:

The site had evidence of recent repairs along the access roadway. There was some rebuilt water bars and new mulch present. There was also a new electric fence installed around the well site, however none of these repairs or installations had a negative affect on the site's stability. The vegetation around the site was performing well, and there were no problems of any consequence. There were no erosional issues apparent at this time, and the site was stable.

2634910 Primary Facility 2017-09-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annuli vent pipes were venting into the same tank. I took a walk around the location and there were no obvious concerns noticed. There was no violation noted.

2729169 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Rag Apple well site. The wells associated with this site are as follows:115-20856, 115-20857, 115-20858, 115-20859. The wells were properly labeled.

The long access road was in good shape and was solid. The pad surface was solid as well. The topsoil pile and cut slope were stable as was the fill slope.

The vegetation on site was green and healthy and there were no erosional issues apparent.

There were no violations.

2743361 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2837057 Primary Facility 2019-02-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of water and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas no detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
