2498620 Primary Facility 2016-07-05 Site Restoration No Violations Noted On 7/5/16, the Department conducted an inspection of the Cabot Macdowall R Pad 1 in response to Cabot’s submittal of a Notice of Termination form for this site. The Department finds that the site is permanently stabilized, and that the record drawings dated 10/3/2011 match the as-built drawings which reflect conditions on the ground as observed by the Department. I recommend this notice of termination be approved.
2554809 Primary Facility 2017-01-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:15p.m., the cellar was about a quarter full of water with no bubbling noticed. The (13x9) vent pipe was open and venting into the production tank, the (9x5) vent pipe was open and venting into the production tank. There was 567psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:35p.m.

2642638 Primary Facility 2017-10-02 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annulus vent pipes were connect together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2748203 Primary Facility 2018-07-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was temporarily shut in. This well was in the AOR for the Haynes wells that are currently being stimulated. There was water 4.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to a production tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2756349 Primary Facility 2018-07-13 Complaint Inspection Pending Today's inspection was conducted as a small spill complaint inspection based on Cabot's self reporting of a release of 10 gallons of produced fluid to the ground. Steve Catalfamo, Cabot EHS Specialist explained that on 7/8/18 at approximately 1620 hours a GDS vac truck was removing produced fluid from the production tanks when a malfunction caused the release of the produced fluid. Photo #2 shows the area where the spill occurred and remediation took place, as evidenced by the new gravel present. GDS conducted an initial cleanup of the material. The following day, Resource Environmental was brought in to characterize the spill. Field screening conducted by resource led to additional soil excavation. Total amount of soil removed for disposal filled 4 drums. The final excavation was approximately 8' by 15' by 4". A remediation summary report will be submitted to the Department for review.
2764459 Primary Facility 2018-08-16 Administrative/File Review Viol(s) Noted & Immediately Corrected Today's administrative inspection has been created to document the receipt of the Remediation Summary Report received by the Department which details the remediation of the small spill which had occurred on July 8, 2018. The Department finds, based on the material presented in the report, that this spill was remediated successfully. No further action is required of COG at this time, as related to this spill.
2851426 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 4' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
