2078607 Primary Facility 2012-06-18 Routine/Complete Inspection No Violations Noted During the inspection it was noted that the rock construction entrance had been installed and appeared to be maintained per the plan. The channels were lined with the double net coir lining. At the time of the inspection, the channel lining was anchored by placing stones along the top edge. While on site the Department requested that the operator consult the engineer who developed the plan for the proper anchoring technique being that a detail was not found in the E&S plan.T he disturbed areas along the access road have been hydroseeded. High visibility safety fence was noted to have been installed to the east of the well site.

2491136 Primary Facility 2016-06-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40 p.m., the cellar was about half full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 793psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:15 p.m.
