The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2439561 Primary Facility 2016-01-11 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45 p.m., the cellar was empty, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 605psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:20 p.m.

2541661 Primary Facility 2016-12-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:00a.m., the cellar was about a quarter full of water with no bubbling noticed. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 496psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:35a.m.

2559181 Primary Facility 2017-02-07 Routine/Complete Inspection No Violations Noted The Department was on site, to conduct a Routine Complete Inspection, in response to the submittal of a Restoration Report. At the time of the Department's inspection, I noted that the topsoil stockpile was still on site. I contacted Mike Murnin (Cabot) who explained that it will remain in place until final restoration.

I noted that there was an area, that appeared to be located within the LOD, being mined on the cut slope. I inquired with Mr. Murnin who explained that this area was in this condition when they began construction on this well site.

2628820 Primary Facility 2017-08-23 Routine/Complete Inspection No Violations Noted The well was in production.

There were no obvious concerns noticed at time of inspection.

2738784 Primary Facility 2018-06-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2834040 Primary Facility 2019-01-31 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was snow and ice 5.5' below grade in the cellar and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
