2089485 Primary Facility 2012-08-02 Drilling/Alteration Pending Patterson 252. Follow up on bubbling on 9 x 13" annulus. On 8/1 after cementing 9 5/8" and waiting on cement for 8 hrs, Operator observed bubbling. Warrior ran a CBL - reportedly some "ratty" spots. haliburton came on site and ran CAST too (more detailed log). At time of inspection making a cut on top of 9 5/8" and instaling H-Pack off. Going to vent and monitor pressure build up and determine if queeze necessary. Compnay Man had annulus filled with water - observed steady bubbling on all sides of 9 5/8". Operator will keep DEP updated on pressure monitoring results.
2089486 Primary Facility 2012-08-06 Drilling/Alteration No Violations Noted Patterson 252 is rig on site. Performed squeeze job at 1404 - 1406' on 8/5. Squeezed a total of .5 bbl cement into perforations. Pressure build on annulus was @ 198 lbs. Currently pressure on 9 x 13" is 6.5 psi. Operator is standby waiting for decision whether to go ahead with drilling. Plan to perform FIT when drill out 9 5/8" shoe. Currently holding 500 psi on 9 5/8" to assist with squeeze. No violations oberved at this time.
2093115 Primary Facility 2012-08-16 Drilling/Alteration No Violations Noted Patterson 252. Drilling to 11415' and cementing by 18th or 19th. Currently have 9 x 13 vented. Had previously completed perf and squeeze job on 9 5/8" casing to eliminate gas migrating to surface through annulus. Have not seen any pressure increase since squeeze job - checked poly tank - no gas vapors observed. Walked site - containment in good condition. Drillers log on site and up to date. No violations observed.

2474494 Primary Facility 2016-04-12 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:40 a.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 756psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:15 a.m.

2814306 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted Frank Hoehle, DEP O&G WQS out of the Scranton District Office and I, arrived at the well pad section of the site at 12:42. The weather conditions are partly cloudy skies, breezy and quite chilly with temps in the upper 20's. Cabot was in the process of performing an expansion of the well pad, and preparing the well pad for drilling operations scheduled to take place in the spring of 2019. Steve Catalfamo, Professional Geologist and Senior Environmental Health and Safety Specialist of Cabot Oil and Gas, accompanied Mr. Hoehle and I during the inspection. The well pad surface was quite muddy due to recent precipitation events and snow melt. There are a total of (4) wells in production on the well pad, the D Williams 1H, 2H, 3H and 4H. I recorded elevated conductivity levels in water in the production fluid tanks containment structure, and water in the skid sump/collection pan beneath the glycol drain as part of the 1H natural gas production unit (GPU).
