2020109 Primary Facility 2011-11-16 Drilling/Alteration Violation(s) Noted DIAZ spud rig on site drilling conductor for 3H. 1H and 2H already set. Have Sensit HXG-3 combustible gas meter on site monitoring gas. Allied cementing conductors - using gas block additive. Permit on site. Site ID sign up. Permit #'s not posted on sign. Violation of 58 P.S. 201(g). Failure to post permit # prior to commencement of drilling.
2026233 Primary Facility 2011-11-28 Routine/Complete Inspection No Violations Noted
2030739 Primary Facility 2011-12-13 Follow-up Inspection No Violations Noted
2077754 Primary Facility 2012-06-13 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Cabot estimated that an approximate 1 bbl. of a synthetic oil based drilling mud was released between the two points. The impacted areas were excavated on 6/9/2012. The excavation around the loose fitting was approximately 7’ x 4’ x 1’ and the excavation around the flare stack was approximately 17’ x 4’ x 2’. Resource Environmental collected a free product sample, two confirmatory samples, and two background samples. Diaz handled the disposal.
2371939 Primary Facility 2015-05-20 Routine/Complete Inspection No Violations Noted
2474487 Primary Facility 2016-04-12 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:40 a.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 751psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:15 a.m.

2813656 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted Frank Hoehle, DEP O&G WQS out of the Scranton District Office and I, arrived at the well pad section of the site at 12:42. The weather conditions are partly cloudy skies, breezy and quite chilly with temps in the upper 20's. Cabot was in the process of performing an expansion of the well pad, and preparing the well pad for drilling operations scheduled to take place in the spring of 2019. Steve Catalfamo, Professional Geologist and Senior Environmental Health and Safety Specialist of Cabot Oil and Gas, accompanied Mr. Hoehle and I during the inspection. The well pad surface was quite muddy due to recent precipitation events and snow melt. There are a total of (4) wells in production on the well pad, the D Williams 1H, 2H, 3H and 4H. I recorded elevated conductivity levels in water in the production fluid tanks containment structure, and water in the skid sump/collection pan beneath the glycol drain as part of the 1H natural gas production unit (GPU). Staining/corrosion is observed on the production fl
