2451736 Primary Facility 2016-01-21 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Cabot L. Burts 3 well site (ESX11-115-0074). The wells asociated with this site are as follows:

This site is located in Forest Lake Township, and its last company E&S inspection was on 1-19-2016. The vegetation on site was very well established and there was no erosion or sedimentation issues at this time.

2709983 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted I inspected the Burts Well Site on March 26, 2018. The access road was a bit muddy where it passes through a quarry, but was solid enough overall. The pad surface was solid and partly snow covered. The cut and fill slopes were well vegetated, visible through the snow. There were no areas of instability noted. The wells were properly labeled, and the wells on site were as follows:
