2121629 Primary Facility 2012-11-14 Incident- Response to Accident or Event No Violations Noted (Violations attached to permit #115-20637) (self-reported release of 200 gallons of glycol)Release on the Corbin 1H was caused by a bad valve inside of the drain on the GPU. The release was found on November 12, 2012 around 11:00AM by a Cabot Well Tender. The Well Tender noted a small drip from the drain. The well tender placed sorbent pads inside the box and tried to tighten the plug.GL Parrish pumped the GPU down in place then began to rig down the plumbing and moved the unit out of the way prior to excavation. GDS began excavating by 2:00PM. A total of 21 cubic yards of material were excavated. The excavation was covered with Dura-Skim liner and wooden rig mats were placed over top for safety. The material was staged on site in a roll off container. Diaz will be handling the disposal. Resource Environmental sampled the excavation and they should have the results by 11/14/12.The Department requested that a copy of the MSDS be sent for the glycol while on site and that the operator submit the sample results within 3 days of receipt.

2472261 Primary Facility 2016-04-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:40 a.m., the cellar had water at the bottom with no bubbling noticed, the (20x13) vent pipe was open and venting to a tank, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 686psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:15 a.m.

The site stability looked good. Only issue observed on the pad as far as production facilities are concerned is the staining located on a production tank which is indicative of an overflow and the relatively high level of storm water present in the secondary containment of the said stained tank. Please explain what your intentions are for the large woodchip pile located on the north side of the pad. If the pile is a waste, please remove.
