Two wells drilled (1H and 2H). 9x5 and 13x 9 annuli vented to poly tank. Eight rolloff containers on site filled with plastic and material scraped off of pad. Some small areas of drill mud on ground. Some of rolloff containers are leaking fluid onto pad. Violations cited at 115-20635 only for residual waste on ground and permit number not posted cited at both wells.
2148868 Primary Facility 2013-03-18 Incident- Response to Accident or Event No Violations Noted An approximate (42) gallons of production fluid were released from the Dobrosielski 1H well. Mr. Webster explained that there were (2) GPUs connected to the 1H well. One of the GPUs was malfunctioning, so the well tender locked it out. It was determined that the well tender forgot to lock out the dump line, so fluid was still flowing through the dump lines which are connected prior to dumping to the production tanks. The production fluid began to back flow to the locked out GPU releasing the production fluid to the surface of the ground on the well pad.

The area was solid and stable with an abundance of vegetation visible through the snow cover. Both the cut and fill slopes were well vegetated and the wells were properly labeled. The wells associated with this site are the 115-20635 and the 115-20636.

There were no violations noted.
