2681654 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted Arrived on location at: 13:00. Weather: Clear, cold ~ 30 degs. On location in response to report of a recent Compressor fire incident reported to the Dept. via email from PEMA. Upon arrival I met with Chief O&G reps. Aaron Stredny, Enviro. Inspector and Margret Farrell, Enviro. Mgr. I was also accompanied by DEP reps. Mathew Shope, WQS and Jonathan Sassi, EPS. Inspection of the US Compressor Unit showed that fire appeared to be localized to the Engine section of the compressor unit. Insulation panels installed to inside of compressor unit enclosure, secondary containment beneath compressor unit and several aluminum siding panels of enclosure were also damaged by fire. Chief had sub-contractors onsite performing clean up and removal of fire debris at time of inspection. Mr. Stredny mentioned that a US Compressor Units rep. had been at site earlier and took pictures of compressor unit for an ongoing investigation as to the cause of fire. DEP reps. Mr. Shope and Mr. Sassi will be performing further inspection a
2768976 Primary Facility 2018-08-20 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1305 hours to conduct a routine complete inspection. The investigation was being conducted in a response to correspondence received from the operator detailing flood/rain damage that has occurred on several of its sites during last week's heavy rains.

On this particular site, the operator identified several areas of concern on site that needed to be addressed:

Washout above infiltration basin one (1) Erosion on berm above infiltration basin two (2) Erosion under matting on berm of infiltration basin three (3) Erosion on access road

At the time of our inspection, all of these issues had been addressed with the exception of the erosion on the access road, which should be repaired promptly. No sediment appeared to be deposited outside the LOD.

Aside from the damaged areas that had recently been stabilized, the slopes and berms on the pad were well vegetated and stable. The pad surface was stable and fairly level with minimal pooling of stormwater. The well heads were prop

2826334 Primary Facility 2019-01-07 Site Restoration No Violations Noted I arrived on site at 1352 hours to perform a site restoration inspection.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, many areas of the pad have not attained 70 percent permanent perennial vegetative cover. In addition the culvert shown in photo 3 is partially buried, there is deposited sediment outside the LOD (photo 5), and the access road contains exposed erodible material and areas of deposited sediment. Operator, please inspect the aforementioned deficiencies and determine if any maintenance should be performed.
