Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1989155 Primary Facility 2011-07-14 Drilling/Alteration No Violations Noted Spudding conductor at time of inspection. Auger Rig - Koda. Site ID sign in place. Crews on site setting up containment and matting. Met with Company man. Permit was not on location at time of inspection. Operator having delivered ASAP. Conductor being drilled to 80' (20"). Should be moving big rig in within the next week or so.
2000273 Primary Facility 2011-08-10 Drilling/Alteration No Violations Noted Company Man Steve Tettleton. Patterson 251. Packer set above shoe at 1808'. @ 1691' to 1713'. Drillers log on site and up to date. BOP charts show good tests. Secondary containment in place and in good condition. Checked cellar - no evidence of gas bubbling. 20" set at 80'. 13 3/8" set at 530' with 30 bbl returns, 9 5/8" set at 1809' with 30 bbl returns.
2005410 Primary Facility 2011-09-15 Routine/Complete Inspection No Violations Noted
2240633 Primary Facility 2014-01-29 Routine/Complete Inspection No Violations Noted
2661214 Primary Facility 2017-11-16 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location for plug and abandonment. Rig cleaned up hole to TD, displace well w/ brine. The rig is currently tripping out, moving uphole to spot cement plug from 7300' - 6800'. Had discussion with the companyman and discussed the job procedure. I conducted a walk-around of the pad. Cementers arrived on location. No obvious issues observed.
2663713 Primary Facility 2017-11-22 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location for P&A. 5 1/2" was recovered from 2480', setting on rack. Several plugs set since last inspection last week, include squeezed perfs in 5 1/2". Updated wellbore diagram attached. I conducted a walk around of location. No obvious issues were observed.
2666523 Primary Facility 2017-12-01 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a well being plugged. Rig is off location. BOP rigged down and tree is back on well. 9 5/8" is being vented through needle valve off 2" iron from A-section. The 13 3/8" has a plate welded to the 9 5/8" to isolate the annulus and a 1" vent coming from the side of the casing, venting through needle valve. These two annulus vents are tied into but isolated from each other. An Altair 5x gas meter detected 100% gas on the 13 3/8" and 36 % LEL on the 9 5/8". There was no visible or audible flow, or pressure felt at the vent. The operator is monitoring the well for a brief period before coming back and finishing the P&A. No obvious major concerns were observed.
2670538 Primary Facility 2017-12-13 Plugging(Includes Plugged/Mined Through) No Violations Noted The rig is back on location to finish plugging of the Jerauld 1H. At the time of inspection, The crew is drilling out cast iron bridge plug at 524' as part of revised procedure to perf and squeeze cement behind 9 5/8" csg to shutoff annular flow. Wireline are spotted in. I spoke with the companyman and discussed the procedure. The companyman anticipates they"ll be through the CIBP later today, perf casing and try for an injection rate. I conducted a walk around of location. No obvious major issues observed.
2673651 Primary Facility 2017-12-21 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine Inspection of a well in process of being plugged. Arrived on location at 10:26 am. Production tree was on well. The rig has moved off location and the operator is monitoring the well following suicide squeeze behind the 9 5/8" casing below the 13 3/8" shoe. It appeared both the wellbore and annulus were both tied into pressure and temperature recorder, though visual access to the well was limited due to blanket and heater on tree. No obvious issues were observed.
2687161 Primary Facility 2018-01-17 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location for P&A currently rigging down after plugging ops. I spoke with the companyman. Window cut in 9 5/8" csg from 520'-540', underreamed to 12 1/4" original wellbore diameter. Packer was set at the bottom of the window, plugged back with cement to 420' yesterday. Pump chart and cement samples looked ok. The crew will rig down, top off well with gravel, install vent as a precaution considering low volume of venting behind the 9 5/8" and mark vent as a well monument. No major issues observed. Wellbore diagram attached.
2687222 Primary Facility 2018-01-24 Routine/Complete Inspection No Violations Noted Rig is off location following plugging ops. Wellbore access was limited-- production tree still on well. The cellar was full of water with no bubbling. There were no obvious issues.
2691230 Primary Facility 2018-02-06 Administrative/File Review No Violations Noted February 6, 2018: Certificate of Well Plugging for the Jerauld 1H (115-20562) has been received, reviewed, signed, and sent to file.
2691312 Primary Facility 2018-01-30 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a plugged unconventional gas well. The well has a 2" candy cane vent installed as a plugged well monument with marker. No gas was detected at the vent, and no obvious issues were observed.
2702895 Primary Facility 2018-03-08 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this recently plugged, unconventional gas well was conducted March 8th, 2018. The well has a 2" candy cane vent with marker installed as a plugged well monument. No obvious issues were observed at this time.
2770380 Primary Facility 2018-09-04 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged, unconventional well was conducted September 4, 2018. At the time of inspection, the plugged well has a 2" candy cane vent with valve and marker plate installed on the well as a plugged well monument. There were no open holes or discharges observed. There was no audible or visible venting observed. No gas was detected with an Altair 5x gas meter at the mouth of the vent. No obvious major issues or concerns were observed during this inspection,
2852626 Primary Facility 2019-03-08 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging inspection of this plugged unconventional well was conducted March 8, 2019. At the time of inspection, the site is reclaimed and a 2" candy cane vent pipe with valve and marker plate is installed on the well as a plugged well monument. The location was snow covered. The snow surrounding the well was clean; no evidence of discharge. There was no visible or audible venting observed. Traces of gas were detected at the mouth of the vent (0-4% LEL). There were no open holes, discharges, bubbling, staining, or other obvious issues observed.
