Currently waiting to get direction from Cabot. Checked cellar. No bubling observed. Casings set = 20" at 90', 13 3/8" at 448', 9 5/8" at 1295'.
1976944 Primary Facility 2011-05-31 Drilling/Alteration No Violations Noted Pioneer Rig 63. PAcker set at 1630. Added 30' sand on top. Squeezed 0.6 bbl of Type 1 cement. Pressured up to 200 lbs. WOC approximately 5 hours. Drilled out cement and completed fit test with 15 lbs pressure for 15 mins. Fit test indicated good seal. Drilled down to 7923'. Currently generator broke down and no drilling. Operator waiting for replacement generator.

Well is shut it and has not been fracced. Vents are open and vented to poly tank. No bubbling observed in cellar. Fluid level below tops of casing strings. No personnel on site. DEP off site at 11:55.
2059471 Primary Facility 2012-04-13 Follow-up Inspection No Violations Noted No activity on site. Well is not online. 13 x 9" and 9 x 5" annulus vented to small poly tank. Can see gas vapors coming from top of tank. No bubbling observed in cellar. Gauge on production string reads 0 psi.

Currently going in hole with stinger - will pump 500 sks Standard A cement into perforations at 5290 - 5282 (4 spf). Setting cement retainer at 5260'. Company man reported they could not break circulation - think when fraccing 8V - frac'd into Stalter 1 and reason for water loss. So plan is to fill void with this cement squeeze. No violations observed at this time.

Tested casing to 2000lbs for 30 mins - had good test. Drilled bottom retainer set at 5455'. Ran in hole to 7500' to CIBP set at 7390'. Tested casing to CIBP to 2500lbs for 30 mins. Tested good. At time of inspection in process of TOH and then will be running bond log. Pressure on 8V seems to be decreasing.

DEP - Steve Watson and Andrea Schmid on site with flow meter. Cabot on site to facilitate collecting data and install pressure recorders on each wells. Readings on Stalter 1H = 13 x 9" - 0 psi with digital gauge, 0 flow with 1/8" plate. 9 x 5" = 0 psi with digital gauge, 0 flow with 1/8" plate.

Cabot installed digital pressure recorders for test. Pressure readings were collected prior to ending test. 13 x 9" annulus showed 1.6 psi, and 9 x 5" annulus showed 43.3 psi. Cabot disconnected recorders and will submit data to DEP. Also collected data from Stalter 2H and 8V.

Fraccing scheduled to start tomorrow morning around 6 am. At this time no violations observed.

Currently shooting perforations for Stage 8. 20 stages total for this well. Pressures on Backsides = 1H - 144 psi (9x5). 8V - 0 psi (9x5), 2H - 11 psi (9x5). Stage 7 High pressure was 8696 psi. 8910 has been highest recorded at this point throughout frac job.

2498679 Primary Facility 2016-07-05 Site Restoration No Violations Noted On 7/5/16, the Department conducted an inspection of the Cabot Stalter D Pad 1 in response to Cabot’s submittal of a Notice of Termination form for this site. The Department finds that the site is permanently stabilized, and that the record drawings dated 10/15/2010 match the as-built drawings which reflect conditions on the ground as observed by the Department. 3 wells have been drilled on this pad, the 1H (115-20517) and 2H (115-20496) both appear to be in production, while the 8V (115-20457) is not tied into production. There are 3 other wells associated with this well pad, the 3H (115-20506), the 4H (115-20497, and the 6H (115-20522), none of which have been drilled. I recommend this notice of termination be approved.
