2226330 Primary Facility 2013-12-03 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Arrived at the site at 10:35 in response to (21) gallon spill of production fluid that impacted the surface of the ground on the Well Pad. The excavation of the impacted area on the ground surface has been completed. Mr. Catalfamo collected (2) confirmation/characterization samples post excavation and (2) background samples on the pad away from the spill area. The gas production unit has been moved in order to clean up the spill. The 2H well has been shut in. The apparent result of the spill was due to a loose hammer union on the GPU. The spill was reported to the Department on 11/30/2013 at 4:30 PM. The spill was discovered around 3:40 PM on 11/30/2013.

The access road and pad surface were stable but had a bit of standing water due to recent rains.

There were no violations noted.
