Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1984074 Primary Facility 2011-06-27 Drilling/Alteration No Violations Noted Pioneer 63. Attempted to cement 9 5/8". Could not get plug to burst. Pressured up to 4500 psi. Pipe filled with @140 sks cement. Will drill out dement with 8 3/4 PDC bit, and then re-cement pipe. Operator to call DEP when re-cementing casing.
1987236 Primary Facility 2011-07-12 Drilling/Alteration No Violations Noted Pioneer 63. Currently drilling on mud in lateral. Drilled on air to 6664'. No misting, soap, or defoamer used during process so far. Initial string of 9 5/8" was pulled, and a new string was installed. Drillers log on site and up to date. Follow up on report of cracked casing complaint. reported downhole issues thus far on well. No violations observed.
2000341 Primary Facility 2011-08-16 Follow-up Inspection Pending Inspected cellar of well. Approx. 1- 1 1/2' of fluid in cellar. Observed small consisten bubbling coming to surface approx 6" from well head. Operator called for pump turck to remove fluid from cellar. Steve Watson and Ryan Klemish returned to site @14:10. Pumped down cellar bubbling appears to be leaking from weld o hanger/conductor. Requested to install gauges on 2H well 20 x13 3/8" and will monitor pressures as part of investigation.
2009308 Primary Facility 2011-10-05 Follow-up Inspection No Violations Noted DEP on site @ 11:45. Follow up on status of well as part of onging gas migration investigation. Well is shut in. Well has not been fracced. Vents are open and vented to poly tank. No bubbling observed in cellar. Fluid level below tops of casing strings. No personnel or activity on site. DEP off site at 11:55.
2027836 Primary Facility 2011-12-08 Follow-up Inspection No Violations Noted No activity on site. Well is currently shut in. Backsides vented to poly tank. Approcximately 1' of water in cellar. No visual signs of bubbling in cellar. Bank of frac tanks on pad. Entrance in gated.
2059461 Primary Facility 2012-04-13 Follow-up Inspection No Violations Noted No activity on site. Well is not online. 13x9" and 9 x 5" vented to poly tank. Gauge on production reads 0 psi. No bubbling observed in cellar.
2106818 Primary Facility 2012-09-25 Follow-up Inspection No Violations Noted On site to perform 72 hour shut in on annular spaces of all wells on pad. DEP - Steve WAtson and Andrea Schmid on site with flow meter. Cabot on site to facilitate data collection and install digital pressure recorders on all annuli. Reading on Stalter 2H at time of inspection: 9 5/8" x 13 3/8" = 0 psi with digital gauge and O flow detected with 1/8" plate. 9 5/8" x 5 1/2" annulus showed 0 psi on digital gauge, and 0 flow with 1/8" plate.
2106876 Primary Facility 2012-09-28 Follow-up Inspection No Violations Noted On site to complete 72 hour shut in test. Started test on 9/25/12. shut in 13 x9" and 9 x 5" annuli. Cabot installed digital pressure recorders on each vent. Pressure readings were collected just before ending test. Readings were= 13 x 9" was 2.9 psi. 9 x 5" was 79.8 psi. Cabot disconnected recorders and will submit data to DEP. Also collected data from Stalter 1H and 8V.
2135086 Primary Facility 2013-01-24 Drilling/Alteration No Violations Noted Met Mr. Edward Lorenzo on site - Co. Man. Currently rigging up to drill out frac plugs and flow back. Will flowback while drilling out plugs so will not require flaring. Pressures on backside are 0 psi. Secondary containment in place. No violations observed.
2244813 Primary Facility 2013-12-11 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Wells shut in 6:57AM, opened at 9AM to obtain samples.

2498680 Primary Facility 2016-07-05 Site Restoration No Violations Noted On 7/5/16, the Department conducted an inspection of the Cabot Stalter D Pad 1 in response to Cabot’s submittal of a Notice of Termination form for this site. The Department finds that the site is permanently stabilized, and that the record drawings dated 10/15/2010 match the as-built drawings which reflect conditions on the ground as observed by the Department. 3 wells have been drilled on this pad, the 1H (115-20517) and 2H (115-20496) both appear to be in production, while the 8V (115-20457) is not tied into production. There are 3 other wells associated with this well pad, the 3H (115-20506), the 4H (115-20497, and the 6H (115-20522), none of which have been drilled. I recommend this notice of termination be approved.
