2737084 Primary Facility 2018-04-17 Follow-up Inspection No Violations Noted The Department was on site to meet with representatives of Chief Oil & Gas to conduct a Follow Up Inspection to the Department's 4/17/2012 inspection which was conducted in response to a self-reported and assumed vandalism incident causing the release of an approximate 500 bbl. of what was reported to be freshwater to the pad's surface. During the inspection conductivity screening revealed elevated readings in the perimeter ditch (Collection Ditch #1) and sediment trap below the location of the release. Additionally, dead amphibians were found within the confines of said sediment trap. A copy of the sample collected during the 4/17/2012 inspection is attached for your reference.

During this inspection, Eric Rooney (DEP-PG) and I met with Mike Walor (Woodard and Curran-Scientist) and Kellon Balum (Chief). At this time, Mr. Walor began to use an auger and a shovel to reach a depth of approximately 28" so that samples collected would be from the base of the sediment trap. The material was soft and water

2822315 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted I arrived on site at 1450 to perform a site restoration inspection.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, the site did not meet 70 percent vegetative cover stabilization requirements for permit termination, and the footprint restored pad did not match the site plan.
