Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1912907 Primary Facility 2010-09-14 Routine/Complete Inspection No Violations Noted No activity on site. conductor set and cellar cemented. Drillers log left in mailbox by Ziegenfuss. Reported 1" stream H20 at 90' and 1 3/4" stream at 115'. Mousehole drilled. Both pipes have metal plates welded over them. Planning on moving in big rig later in week.
1914993 Primary Facility 2010-09-21 Drilling/Alteration No Violations Noted Patterson 251. Drilling 12 1/4" hole to 1800' and then setting 9 5/8". Drillers log on site and up to date. Secondary containment in good condition. No pit- using closed loop system. No violations observed.
1918668 Primary Facility 2010-10-07 Drilling/Alteration No Violations Noted Pat. Rig 251. Drling depth @ TD = 10646', Setting 5 1/2" at 10613'. Haliburton on site to do cement job. Will bring cement back up into surface string. Have Ziegenfuss coming on site tomorrow to spud 2H well.
1973714 Primary Facility 2011-05-13 Routine/Complete Inspection Violation(s) Noted
2016187 Primary Facility 2011-11-02 Drilling/Alteration No Violations Noted Complete Fluid Control on site. Performing clean out run. Drilling out ice-plugs. Only well on site that has been fracced so far. Secondary containment in place. No violations observed.
2035225 Primary Facility 2012-01-17 Routine/Complete Inspection No Violations Noted
2062242 Primary Facility 2012-04-17 Incident- Response to Accident or Event Pending
2316775 Primary Facility 2014-10-16 Complaint Inspection Pending The Department (Andrea Mullen, Eric Rooney, Jon Sassi, and I) met with Jon Hafer (Chief Oil & Gas) on site at the Squier Unit Well Site to collect soil samples in response to a nearby water quality complaint. The Department collected the below referenced samples to be analyzed with the SAC 961 with the addition of sulfates, lead, calcium, bromide dry weight, and lithium :
2549606 Primary Facility 2016-01-04 Routine/Partial Inspection No Violations Noted
2729092 Primary Facility 2018-04-17 Follow-up Inspection No Violations Noted The Department was on site to meet with representatives of Chief Oil & Gas to conduct a Follow Up Inspection to the Department's 4/17/2012 inspection which was conducted in response to a self-reported and assumed vandalism incident causing the release of an approximate 500 bbl. of what was reported to be freshwater to the pad's surface. During the inspection conductivity screening revealed elevated readings in the perimeter ditch (Collection Ditch #1) and sediment trap below the location of the release. Additionally, dead amphibians were found within the confines of said sediment trap. A copy of the sample collected during the 4/17/2012 inspection is attached for your reference.

During this inspection, Eric Rooney (DEP-PG) and I met with Mike Walor (Woodard and Curran-Scientist) and Kellon Balum (Chief). At this time, Mr. Walor began to use an auger and a shovel to reach a depth of approximately 28" so that samples collected would be from the base of the sediment trap. The material was soft and water

2741763 Primary Facility 2018-06-15 Administrative/File Review No Violations Noted On 5/16/2018 the Department received an email from Richard Adams regarding the sample results from the 4/17/2018 sampling event at the Squier A Well Site. Chief Oil & Gas submitted a Field Sampling Plan to the Department on October 4, 2017 detailing their plan. Strontium and chloride were non-detect in any of the four samples (excluding background samples). Specific conductance was measured between 118-572 in the four samples associated with the sediment trap. Barium was detected below the MSC for soils at all sample points. At this time, the Department acknowledges the sample results and will resolve the pending inspection dated 4/17/2012.
2822312 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted I arrived on site at 1450 to perform a site restoration inspection.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, the site did not meet 70 percent vegetative cover stabilization requirements for permit termination, and the footprint restored pad did not match the site plan.

No violations were noted.

2822870 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted I arrived on site at 1450 to perform a site restoration inspection.

After performing an administrative review of the plan and a site inspection, I do not agree that site conditions meet the requirements for the termination of this permit. Namely, the site did not meet the 70 percent perennial vegetative cover stabilization requirements for permit termination, and the footprint of the restored pad did not match the site plan.
