I took a walk around the location and everything looked OK.

I left the site at 12:25 a.m.

The site was well vegetated with the correct perennial grasses, over the 70 percent perennial vegetative threshold and the earthwork was performed to specifications. There were no erosion or sedimentation issues with this site.

2548706 Primary Facility 2016-12-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:30p.m., the cellar was full of ice. The (13x9) vent pipe was open and venting into the production tank, the (9x5) vent pipe was open and venting into the production tank. There was 557psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:15p.m.

2636146 Primary Facility 2017-09-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annuli vent pipes were connected together and venting into the same tank. I took a walk around the location and there were no obvious concerns noticed.

2728175 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection for the Jo Blaisure pad. The wells associated with this pad are as follows: 115-20239, 115-20245, 115-20399, 115-20400, 115-20401. The wells were all properly labeled.

The access road and pad surface were solid and stable with no erosional issues.

The cut and fill slopes were stable and well vegetated and were greening up nicely. There did not appear to be any erosional issues.

There were no violations noted.

2743792 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank at 100% LEL and 100% methane. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2838588 Primary Facility 2019-02-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 2' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
