I took a walk around the location and everything looked OK.

I left the site at 13:10 p.m.

2549911 Primary Facility 2017-01-05 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination (NOT) for the W Hawley P1 well site (Erosion and sediment permit 58-09-802). The wells associated with this site are as follows:


The site was well vegetated with the correct perennial grasses, over the 70 percent perennial vegetative threshold and the earthwork was performed to specifications. There were no erosion or sedimentation issues with this site.

There were no violations

I recommend approval of the NOT for this site.

2809088 Primary Facility 2018-11-29 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the W. Hawley pad during a FUI visit to assess a release which occurred onsite previously.
    The weather at the time of inspection was overcast and about 34 degrees.  There had been some light overnight snow.  
    The slopes surrounding the site were well vegetated, visible through the approximately four inches of snow on the ground.  There had obviously been a heavy crop of goldenrod on the cut slope during the summer, and there were quite a lot of raspberry/blackberry bushes occupying the fill slope.  Below the fill slope there was a small brushhogged field.  There were no ruts, slides or erosional issues apparent on any of the areas surrounding the pad at the time of inspection.
    The access road and pad surface were solid and stable.  There were two rolloff containers staged onsite for use in the spill remediation.
    The wells were properly labeled. There were no erosional issues noted during the inspection.
    There were no violations noted.
2810532 Primary Facility 2018-11-27 Incident- Response to Accident or Event No Violations Noted I arrived at the site 13:10 in response to a reported spill incident of an estimated (30-40) gallons of glycol from a natural gas production unit (GPU) to the pad surface. The weather conditions are cloudy skies, with a light snow shower, breezy and cold with temps in the low to mid 30's. Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas, and Eric Castonguay, Project Geologist of Woodard and Curran were at the site during the inspection. The Cabot subcontractor, GasSearch Drilling Services (GDS) is in the process of removing storm water accumulated from recent precipitation events in the area where the spill impacted the pad surface. GDS had recently completed remediation of the impacted area via a backhoe. The W Hawley 2H well is in production. The W Hawley 1H well is currently in production.

I had contacted Mr. Watson at 9:48 on 11/27/18 to inquire of the specifics of the incident. Mr. Watson provided me with the following information during the phone conversation. "A w
