2139940 Primary Facility 2013-02-14 Follow-up Inspection No Violations Noted On site to collect pressure readings from annuli of well. DEP reps on site = Steve Watson, Eric Rooney, and Gene Rickard. DEP met with John Smelko of Cabot. Selman reps on site to assist with collection of pressure data. Cabot shut in annuli at @ 9am this morning. Cabot installed a digital gauge on the 16" x 9 5/8" annulus. Pressure was 0 psi. Gauge was removed and open vent flow was screened using a Model 60 gas meter. Model 60 reading = 0% cobustible gas. Gauge was then installed on 9 5/8" x 5 1/2" annulus. Pressure = 1 psi. Model 60 reading = 100 % combustible gas. No samples were collected. After completion of testing, Cabot re-conected vent lines and locked vents open.
2481924 Primary Facility 2016-04-27 Routine/Partial Inspection No Violations Noted The Department was on site at the Greenwood, R. Pad 1 to conduct a Routine Partial Inspection. The E&S Inspection Log indicated that Mike Brilla (TEC) conducted a Post Rain Event Inspection on 4/27/16. It appears as though the landowner has made a road to the left of the access road gate to circumvent said gate. Also, it seems as if the landowner has been working areas on the well site. There is a large area within the well pad surface that looks like wood chips have been strewn across it and sections of plastic packaging are intermixed.

The perimeter ditch does not appear to meet temporary stabilization requirements. The Department requests that the operator investigate the area and deploy a method of erosion control that is consistent with your plan. The production tanks were stained during the inspection. One of the tanks has more staining than the other. The Department requests that the operator inspect the tanks to find the origin of the staining.

2642428 Primary Facility 2017-10-02 Site Restoration No Violations Noted I inspected the site for notice of termination (NOT) on October 2, 2017. The site was surrounded by active agriculture (field corn). The access road leading in was in good shape with no rutting and the site was well vegetated on the cut and fill slopes. The pad surface was solid and stable as well. There were no erosional issues noted.

I recommend APPROVAL of this NOT.

There were no violations observed.

2774791 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted Arrived on location at 09:00. Weather: ~ 65 degs., light rain, slight breeze. Onsite to initiate 72 hr. pressure build up test "PBUT" on the annuli of the Greenwood 1V, 6H, 7H & 8H wells. All wells, back side of production casing annuli have been isolated and were venting to onsite poly vent tanks. The 6H, 7H & 8H wells (16"x 9 5/8") cemented annuli had open top 16" casings in cellar *. The 1V wells (9 5/8"x 7") cemented annulus was beneath water level in cellar, no bubbling was noted at time of inspection.

- Upon arrival I met with Cabot O&G Reps. Jeremy Rapp and Sully Rutledge. Prior to shutting in wells annuli, initial gas screenings and flow checks were performed at the 1V (7"x 4 1/2") and 6H, 7H & 8H (9 5/8"x 5 1/2") annuli using the Depts. Altair 5X gas meter, calibrated this morning, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed: - 1V: (7"x 4 1/2") = 100% CH4, 9.2 cf/d; (9 5/8"x 7") open casing top in cellar, No bubbling noted. - 6H: (9 5/8"x 5 1/2") = 100% CH4, 15.2 cf

2775557 Primary Facility 2018-09-14 Routine/Complete Inspection Pending Arrived on location at: 09:00. Weather: ~ 75 degs., Light rain, Slight breeze. Onsite to conclude the 72-hr. pressure build up test ¿PBUT¿ being performed on the Greenwood 1V, 6H, 7H & 8H wells, back side of production casing annuli and the 1V¿s (9 5/8¿x 7¿) cemented annulus.

- Upon arrival I met with Cabot O&G Rep. Sully Rutledge and Selman and Associates Reps. Kaydee Oliver and Russell Mohlt. There was also a GDS Vac Truck and Operator onsite to add or remove water to cellar areas as needed. The ending PBUT pressures were checked by Cabot Rep. using a digital Crystal pressure gauge. Ending pressures are listed below: - 1V: (7"x 4 ½¿) = 21 psi., Gas sample taken = 100% CH4; (9 5/8"x 7") = 0.0 psi., No sample taken. Cellar was initially full of water w/ no bubbling noted. Cellar was then emptied confirming (9 5/8"x 7") vent line connection to enclosed casing top. - 6H: (9 5/8¿ x 5 ½¿) = 12.0 psi. Gas sample taken = 100% CH4; (16"x 9 5/8") = 0.0 psi. 2804931 Primary Facility 2018-11-15 Follow-up Inspection Pending Arrived on location at 11:00. Weather: Overcast, ~ 32 degs., Snow forecast for this afternoon. Onsite to perform initial gas flow checks at the 8H wells annuli, prior to starting the follow-up 72 hr PBUT on this well. At conclusion of the initial PBUT on 9/14/18, bubbling gas was noted to inside of the open top (16"x 9 5/8") annulus. Since the last PBUT, Cabot has capped off and contained this annulus in preparation for this follow-up PBUT. (* See Pic #1.)

Upon arrival I met with Cabot O&G Rep. Jeremy Rapp. Also present were DEP Reps. Eric Rooney, Geologist and Casey Baldwin, WQS. Initial gas screenings and flow checks were performed using the Depts. Altair 5X gas meter, calibrated this morning, and Cabots' RS-484 Alicat "Whisper" flow meter. Pre-shut in screenings showed: - 8H: (16"x 9 5/8") = 0% CH4, 0.0 cf/d; (9 5/8"x 5 1/2"") = 100% CH4, 12.8 cf/d.

Onsite for conclusion of the follow-up 72 hr PBUT on the 8H wells annuli. At conclusion of the initial PBUT on 9/14/18, bubbling gas was noted to inside of the open top (16"x 9 5/8") annulus. Since the last PBUT, Cabot has capped off and contained this annulus in preparation for this follow-up PBUT. (* See Pic #1.)

Upon arrival I met with Cabot O&G Rep. Jeremy Rapp. Also present was DEP Rep. Eric Rooney, Geologist and Selman & Assoc. Rep. Tyler Thomas. The ending PBUT pressures were checked by Cabot Rep. using a digital Crystal pressure gauge. Ending gas checks were performed using the Dept's Altair 5X gas meter, calibrated on 11/15/18. Ending pressures and gas screening results are listed below:
