Broken sight glass for Glycol storage tank causing release of Glycol to ground around gpu. Due to rain, spill migrating from gpu to pooled rain water on pad. DEP on site at 13:30. At time of arrival, gpu covered with large tarp. Raining steadily. Visible sheen on water. Cause of spill may be from bullet hitting site glass. Evidence reportedly found on site suggests this. According to Operator gpu holds @500 gallons of Glycol -estimated release +/- 100 gallons. Crews on site installing absorbent booms and pads. Backhoe on site creating earthen berm to confine spill. Vac truck is on way. State Trooper arrived on site to investigate incident. Appears to be vandalism at this time. Operator will clean up spill following Act 2 Guidelines and report to DEP. Act 2 ROL granted 12/18/12, MBC data clean-up 3/25/15.
2373089 Primary Facility 2015-05-18 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected NERO EC&B granted Act 2 ROL 9/9/15. Self-report of a release of production fluid. It was reported that an estimated 10 gallons of production fluid were released to the well pad surface on 5/7/15. A slug/burp of production fluid exited the hatch on the top of a production fluid tank in the form of a mist as a Cabot well tender was in the process of performing maintenance on the Grosvenor 1H well. The well tender was adding soap sticks into the well to decease the hydrostatic pressure of the water in the well.

2484338 Primary Facility 2016-05-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:15 p.m., the cellar was full of water with no bubbling noticed, the (20x13) vent pipe was open and venting to a tank, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 496psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:40 p.m.

2487884 Primary Facility 2016-05-26 Site Restoration No Violations Noted Inspection of the Grosvenor D P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices were in place according to approved plans and appear to be functioning properly.

Based on the site inspection, I recommend this notice of termination be APPROVED.
