During drilling of surface pipe, has cuttings blow beyond the reserve pit onto adjacent bank. A hole was noticed in the liner of the pit, and springwater was flowing into pit. Operator in process of sucking out all cuttings and placing in temporary pit. Plan is to remove liner, and re-line reserve pit. I mentioned that the base of pit needs to be above seasonal high water table. Cabot using URS to determine watertable.
1879751 Primary Facility 2010-04-20 Drilling/Alteration No Violations Noted Co. Man notified DEP that possible poor cement job on surface casing, based on CBL run. Planning on squeezing cement at @ 440'. Have bubbles coming to surface between surface pipe and conductor. 9 5/8" set at 1500'. Cemented on 4/8/10 with 100 sks lead (11.5 lb) and 609 sks tail (14.8 lb). 40 bbls cement return reported.

Co. man on site said they perf'd casing and could not squeeze any additional cement into backside. Drilling has continued at this point. Operator stil discussing posible solutions to stop surface bubbling. Mud loggers provided DEP with copy of mud log.
1879781 Primary Facility 2010-04-22 Follow-up Inspection No Violations Noted Jay Theriot on site- Co. Man. Drilling on air - depth at 4785'. Planning to set plug at 440' with 30' of sand on top. Plan to use 24 gram shot to do second perforation of 2' section of 9 5/8" casing. 4 shots/ft for total of 8 shots. Top of perf- 388, bottom at 390. Will then try to squeeze add'l cement into backside. Have service companies coming to site to run gamma log and CBL.
1879792 Primary Facility 2010-04-25 Follow-up Inspection No Violations Noted Met with Bruce Jankura-DEP. Co. Man on site - Jay Theriot. Well is pressured up to 2400 lbs. Checked gauge while on site and has bled off to 1900lbs. Additional cement was squeezed downhole. As of 6:00 pm on 4/24, bubbling has stopped in cellar. Operator continues to monitor for bubbling. At 3:00 pm today, plan is to drill out and set plug to run another CBL, and possibly additional logs if necessary. There was no bubbling in cellar at time of inspection.
1879794 Primary Facility 2010-04-27 Follow-up Inspection No Violations Noted Co. man on site Jay Theriot. Completed CBL as of 3:00 am today. At time of inspeciton there was no bubbling in cellar. Crew was TOH and continue to monitor cellar.

(52'). superior well services on site to provide cementing services. plan to use 1" trmie to add cement in between casing and then WOC at least 4 hours. Will call DEP upon completion of cementing to update status of well.
1881367 Primary Facility 2010-05-03 Drilling/Alteration No Violations Noted Pioneer Rig 63. TOH and putting hamer bit on to resume drilling. On 4/30, superior pumped cement in between conductor and surface pipe using 1" tremie pipes. Pumped 60 sks of cement and had returns throughout entire job. Cement was in place at 17:00 hours. Driler stated they waited on cement approximately 14 hours before proceeding. Bubbling has stopped in cellar.

I inspected cellar of well and did not see any evidence of bubbling. There was standing water in the cellar at the time of inspection. I asked the company man to notify us if any bubbling became present in the future. Freeboard in the reserve pit was @4'. The liner appeared to be in good condition. The site was lined with plastic. No spills observed. No violations cited.

2244114 Primary Facility 2014-02-04 Incident- Response to Accident or Event Violation(s) Noted Inspection conducted with Brody Webster on 2/4/2014 of the Berry, D. Pad 1 small spill which was identified on 2/1/2014 and called in by Brody Webster on that same day at 5:30 AM. The spill occurred when a dump line froze which caused a hammer joint to loosen and subsequently release production fluid to the ground. Reported spill volume was 42 gallons. Excess liquid was removed with a vac truck. The impacted site has been covered with plastic. All dump lines are covered with plastic which, when used in conjunction with external heaters, is used to thaw out the dump lines. NERO EC&B Act 2 ROL granted 6/30/14, closed.

I took a walk around the location and everything looked OK.
