1852135 | Primary Facility | 2009-12-23 | Drilling/Alteration | No Violations Noted | Arrived on site at approximately 9:00am. I met with Company Man on site Jay Theriot. At the time of inspection, operator was drilling a 12-1/4" hole on air to a depth of 1500' and then will set 9-5/8"surface casing. The Company Man indicated that cementing of the surface casing would likely occur later on this day. The drillers log was on site and up to date. The area surrounding the substructure was lined with plastic and bermed. E/S controls were in place. The reserve pit had adequate freeboard and the liner appeared to be in good condition. |
1853145 | Primary Facility | 2009-12-31 | Follow-up Inspection | No Violations Noted | It was reported that 30 ¿ 40 gallons of diesel fuel spilled onto the plastic liner located directly underneath the tank. Mr. Rogier stated that all of the diesel fuel was contained and none had reached the ground. At the time of inspection, there was no visible evidence of diesel spilled beyond the secondary containment. The cause of the spill was human error. A valve that was supposed to be closed during filling was left open, and subsequently, too much fuel was pumped into the tank. The spilled diesel fuel was cleaned up with absorbent rags and squeegees and was placed in containers for disposal.Based on my inspection, the spilled diesel fuel was contained in secondary containment already in place at the site. There was no evidence of diesel fuel spilling beyond the secondary containment onto the ground. No violations were identified during this inspection. |

1873022 | Primary Facility | 2010-03-24 | Follow-up Inspection | No Violations Noted | Changing out motor and bit. TOOH. Follow up on cuttings spill. Area has been cleaned up. Site ID sign has also been installed. Previous violations have been corrected. |
2107030 | Primary Facility | 2012-10-15 | Drilling/Alteration | No Violations Noted | Frac completed ,Baker hues verbally reported no problems through 20 stages , 0 lbs. pressure on the 1-v plugged well on site |
2107034 | Primary Facility | 2012-10-12 | Drilling/Alteration | No Violations Noted | Site in good shape containment sufficient preparing to frac 12/20 stage ,collected records for previous stages. |

2110142 | Primary Facility | 2012-10-10 | Drilling/Alteration | No Violations Noted | On site to perform Pre-frac inspection. Met with Larry Fulmer - Completions Supt. SPCC and MSDS's on site. WEnt through Engineering Study submitted by Cabot (9/24/09) and confirmed each item was present and implemented as per study. Based on my review and inspection all items proposed by Cabot in the document are in place. mr. Fulmer indicated that they will be performing a 20 stage frac, and fraccing operations will occur 24/7. At this time no violations were observed. DEP offsite 15:30. |
2112270 | Primary Facility | 2012-10-17 | Drilling/Alteration | No Violations Noted | hooking up to gathering line location and pad in acceptable condition, water sully lines are being taken up |
2390909 | Primary Facility | 2015-07-20 | Routine/Complete Inspection | No Violations Noted | |
2487298 | Primary Facility | 2016-05-24 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 8:50 a.m., the cellar was about three quarters full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 590psi on the production casing. I took a walk around the location and everything looked OK.
I left the site at 9:10 a.m. |
2641294 | Primary Facility | 2017-09-28 | Site Restoration | No Violations Noted | I inspected the Baker pad on September 28, 2017 for Notice of Termination (NOT). The site's access road was smooth and solid, with a cornfield bordering the one side. The site itself was well vegetated and nicely maintained, mowed neatly and compact in size. The vegetation was all healthy and exceeded the 70% perennial vegetative threshold. The pad surface was smooth and not rutted and there were no erosional issues present at the time of inspection.
I recommend APPROVAL of this NOT. There were no violations noted |
2672705 | Primary Facility | 2017-12-19 | Administrative/File Review | No Violations Noted | I approved a restoration report on December 19, 2017 for this well site. |
