1852135 Primary Facility 2009-12-23 Drilling/Alteration No Violations Noted Arrived on site at approximately 9:00am. I met with Company Man on site Jay Theriot. At the time of inspection, operator was drilling a 12-1/4" hole on air to a depth of 1500' and then will set 9-5/8"surface casing. The Company Man indicated that cementing of the surface casing would likely occur later on this day. The drillers log was on site and up to date. The area surrounding the substructure was lined with plastic and bermed. E/S controls were in place. The reserve pit had adequate freeboard and the liner appeared to be in good condition.
1853145 Primary Facility 2009-12-31 Follow-up Inspection No Violations Noted It was reported that 30 ¿ 40 gallons of diesel fuel spilled onto the plastic liner located directly underneath the tank. Mr. Rogier stated that all of the diesel fuel was contained and none had reached the ground. At the time of inspection, there was no visible evidence of diesel spilled beyond the secondary containment. The cause of the spill was human error. A valve that was supposed to be closed during filling was left open, and subsequently, too much fuel was pumped into the tank. The spilled diesel fuel was cleaned up with absorbent rags and squeegees and was placed in containers for disposal.Based on my inspection, the spilled diesel fuel was contained in secondary containment already in place at the site. There was no evidence of diesel fuel spilling beyond the secondary containment onto the ground. No violations were identified during this inspection.

2487298 Primary Facility 2016-05-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:50 a.m., the cellar was about three quarters full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 590psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 9:10 a.m.

2641294 Primary Facility 2017-09-28 Site Restoration No Violations Noted I inspected the Baker pad on September 28, 2017 for Notice of Termination (NOT). The site's access road was smooth and solid, with a cornfield bordering the one side. The site itself was well vegetated and nicely maintained, mowed neatly and compact in size. The vegetation was all healthy and exceeded the 70% perennial vegetative threshold. The pad surface was smooth and not rutted and there were no erosional issues present at the time of inspection.

I recommend APPROVAL of this NOT.
