2095596 Primary Facility 2012-08-22 Follow-up Inspection No Violations Noted Follow up on fire involving production units. Incident occurred on 8/21/12. At time of inspection no personnel on site. Based on my investigation it appears sand washed out one of the dump lines exiting the unit for the 1H well. There is a visible hole in the pipe and evidence of damage from high pressure sand on outside of shell to unit. Fire damage visible on productions units for 1H and 2H. The plastic liner of the containment in the area of the fire has been melted. There doesnt appear to be any spilled material outside containment or on ground. There is visible burning of grass on back near back of production units. All well on site have been shut in. At this time waiting for detailed report from Operator on incident. DEP offsite at 8:45 am.

2713347 Primary Facility 2018-03-20 Routine/Complete Inspection No Violations Noted Arrived onsite at 14:30. Weather overcast, ~ 38 degs. (5) wells were present on pad, the Phelps 2H, 1H, 1V, 3H and 4H. The 2H well was shut in and the other (4) wells were producing at time of insp. There was also a US Compressor unit onsite. The 1V, 1H & 2H wells were connected to compressor unit at time of insp. Visual insp. of production tanks showed moderate to heavy corrosion and rust staining at 3 out of 6 prod. tanks. (See pics on page 2). Updating and/or maintenance of prod. tanks is needed.

- Inspection of wells showed all (5) wells had the prod. tree top press. "PTTP", prod. casing press. "PCP" and prod. line press. "PLP" being monitored by wireless data sending units and/or analog gauges. - The 2H, 1H & 1V wells had (1) annular vent present labeled (9 5/8"x 5 1/2") venting to a shared poly vent tank. - The 3H & 4H wells had (2) annular vents present labeled (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") venting to a shared poly vent tank. A gas check performed at this vent tank lid opening showed 100% C
