2496278 Primary Facility 2016-06-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:00a.m., the cellar was full of gravel with no gas detected at the perimeter of the cellar, the (9x4) vent pipe was open and venting to a tank, there was 557psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:35a.m.

2516838 Primary Facility 2016-09-01 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination (NOT) for the Shields G P2 well site, permit number 58-09-802. The wells associated with this site are as follows:

The site was constructed according to plan, the vegetation was perennial, at or above the 70% vegetative standard and there were no erosional actions occuring at this time. All BMPs have been removed. I recommend APPROVAL of this Notice of Termination.

2705256 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted Arrived at location at 09:45. Weather: Overcast, cold, ~ 25 degs. (2) wells were present on location, the Shields, G. 2H and 4H. Both wells were producing at time of inspection. Both wells production pressures were being monitored by remote monitors sending data to Total Flow Control Units "TFCU"s with LED readouts located by production units. Each well had (1) annular vent plumbed to a shared poly vent tank. Both vents were labeled (9 5/8" x 5 1/2") and were open and venting to vent tank. A gas check performed at the shared vent tank opening showed: 100% LEL, 100% CH4. Gas check performed 3' away from vent tank showed 0% LEL, 0% CH4.

- Review of well history showed that a previous 72 hr. pressure build up tests "PBUT" had been performed on these wells on (10/29 - 11/1/11). The PBUT showed the 4H, (9x5) press. built to 166 psi. and the 2H, (9x5) showed no press. build up. - Further review of well history revealed that these wells are part of an ongoing gas migration investigation initiated by the Dept. on 1

2709444 Primary Facility 2018-03-19 Routine/Complete Inspection No Violations Noted Arrived at location at 09:10. Weather: Clear, cold, ~ 30 degs. (2) wells were present on location, the Shields, G. 2H and 4H. Both wells were producing at time of inspection. Onsite to witness start of the 72hr. press. build up test "PBUT" being performed on both wells surface x production casing annuli. Upon arrival met with Cabot O&G Reps., Adam, Aaron, Brody and Steve onsite. Both wells surface x production casing annuli were equipped with pressure logging units tied into Cabot's cygnet monitoring system. Adam confirmed communication with loggers through cynet system. These annuli were shut in at 09:15. We will return to site on Thursday, March 22 at 09:15 for conclusion of the 72 hr. PBUT.

Wells pressures were taken off LED readouts mounted at production units. (Prod. Casing Press. = CP, Static Press. = SP, Differential Press. = DP.) Shields, G. 2H: CP = 461.6 psi., SP = 474.4 psi., DP = 25.2 psi. Shields, G. 4H: CP = 473.6 psi., SP = 476.2 psi., DP = 92.4 psi.

2710033 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted Arrived at location at 09:45. Weather: Clear, cold, ~ 30 degs. Onsite for the conclusion of the 72hr. press. build up test "PBUT" being performed on both wells surface x production casing annuli. The PBUT was started on 3/19/18 at 09:15. Upon arrival met with Cabot O&G Reps., Adam Vogt & Aaron Onks and (3) Selman & Assoc. Reps. Both wells surface x production casing annuli were equipped with pressure logging units tied into Cabot's cygnet monitoring system. The PBUT ending pressures were:

- 2H: (9 5/8"x 5 1/2") = 49 psi. - 4H: (9 5/8"x 5 1/2") = 84 psi. Gas checks performed by the Dept. and Selman showed: 100% CH4. The Dept. and Selman collected gas samples off both these annuli at approx. 09:50. Both wells annuli were bleed down and opened back up to poly vent tank. Post PBUT, water samples were collected at area resident by Eric Rooney, D.E.P. Geologist and Moody & Associates prior to bleeding pressures off of gas wells. - Review of well record for the 4H well showed the 5 1/2" prod. casing cement top was
