Mr. Savage further indicated that they were currently stuck at depth and were washing out the hole with mud. Once the hole is cleaned out and drill pipe removed, 5-1/2¿ production casing is to be set at approximately 7300¿.

2490186 Primary Facility 2016-06-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:55 a.m., the cellar was full of gravel with no gas detected at the perimeter of the cellar, the (9x5) vent pipe was open and venting to a tank, there was 522psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 11:35 a.m.

2493156 Primary Facility 2016-06-15 Site Restoration No Violations Noted Inspection of the Elk Lake School District P1 well pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. The restored pad size is consistent with the PCSM/Site Restoration Plan. Post construction storm water management devices were in place according to approved plans and appear to be functioning properly.

Based on the site inspection, I recommend this notice of termination be APPROVED.

2674276 Primary Facility 2017-12-22 Administrative/File Review No Violations Noted I approved a restoration report on December 19, 2017 for this well site.
