Mr. Theroit reported that at 4075¿ two collars and a hammer bit were lost downhole. The hole was cemented back up to 3430¿ using 173 sks of Class A cement. An open-hole sidetrack is being run around the lost tools down to approximately 6200¿. Depth of drilling at the time of inspection was 3818¿.

Plastic has been placed down on the pad. Drilling was set to begin in a few days from time of this inspection.

BOP¿s were in place and tested. Freeboard in the reserve pit was approximately 10¿. The pit liner appeared to be in good conditions with no tears visible.

We met with Joe Christensen, Data Specialist/Chemist of Comtech Industies Inc. Mr. Christensen informed us that this site is mainly utilized as a storage site for frac water, mainly from the first stages of flowback, along with polymer water used to lubricate the drill bit. The site services flowback waters from multiple Cabot Oil and Gas well sites. The flowback waters are stored in frac tanks at the site and transported to the Hibbard Flowback Site operated by Comtech Industries Inc. for treatment and intent for beneficial reuse at multiple Cabot Oil and Gas Well sites for well completions. Currently, close to (70) frac tanks are housed at this Well Pad. There were several small areas of ponded water on the Well Pad containing petrloeum like sheens. The Department recommended the sheens be removed from the ponded water at the site. Departed from the site at 13:45.
